After Ray, my life partner, was diagnosed with dementia and Parkinson’s disease, he lived in a nursing home. Ray was unable to manage making calls on his own, so the only time we could talk to one another was when a nurse had time to help him. Not being able to touch Ray, see him, or feel confident that he was receiving the best care left me feeling helpless. When my friend, a social worker, suggested I contact Hospice & Palliative Care Buffalo (HPCB) to see if a nurse could visit Ray, I replied, “But he isn’t dying,” mistakenly believing that hospice was only available for end-of-life care. I have since learned that HPCB also provides palliative specialized medical care for individuals living with a serious illness, regardless of where they reside, including a nursing facility, hospital, or their own home. Kelley Clem, HPCB’s Vice President of Advocacy and Education, says, “Ray was a whole human being, rather than a medical algorithm. The number of institutionalized people with IDD has been declining for decades, yet many people with IDD still live in settings with seven or more people. A recent Policy Research Brief found that the use of large group facilities varies widely across states, and many facilities are not licensed or regulated. People live better in small settings than in institutions, so this brief recommends downsizing or closing large institutions and limiting the use of nursing homes and psychiatric facilities to house people with IDD. https://z.umn.edu/a3uv

Understaffing in nursing homes has been a huge problem, impacting care and frustrating families. Next Step Health Care, operator of 16 nursing homes across Massachusetts was ordered to pay $4 million in a settlement with the Attorney General's office, for understaffing leading to harm and neglect. A monitor will be in place at 8 of the facilities. In full practice authority states, NPs can help meet patients’ needs in underserved communities — but not Pennsylvania. By placing practice restrictions on our valuable NP workforce, Pennsylvania is missing out on the many proven benefits of full practice authority: 🏥 Full practice authority would increase direct patient care by a full additional week of access — that's the equivalent of 109 additional patients per NP each year! 🏥 Full practice authority laws reduce care-amenable deaths by 12 per 100,000 individuals, with the largest reductions in rural areas. 🏥 Diabetic patients receive better, less costly care after their states lift practice restrictions on NPs, with rural patients seeing the most significant benefits. Evidence suggests that individuals who receive live-in care are less likely to experience falls and significantly less likely to suffer a hip fracture compared to those in residential and nursing homes. Previous research commissioned by The Live in Care Hub shows that with live-in care, you are less likely to have a catastrophic hip fracture than if you are in a care or nursing home. In fact, you are twice as likely to do so in a nursing home than when receiving care in your own home. In a normal year, there are about 66,000 hip fractures a year in England, Wales and Northern Ireland. According to the Royal College of Physicians, “hip fracture patients face a significant risk of dying or losing their independence.” Tragically, a month after suffering a hip fracture, one in 12 people will sadly pass away and only half will have returned home.
