The African digital health summit will be a great occasion to foster our findings in digital health education for therapeutic adherence. some details : we have found a digital education method to improve on therapeutic adherence of patients with chronic diseases like hypertension but also HIV or Diabetes etc, this can also be applied to reduce vaccin hesitancy in populations facing epidemics and to improve good participatory practices during clinical trials. we know how it is important to keep patients adhering to long treatments to avoid challenging complications, or vaccins introduced to be well taken by target populations. What amazing discussion we had yesterday with a group of outstanding global experts from across the world. The overriding challenge we were discussing was health equity in Sickle Cell Disease- the most prevalent group genetic disorders globally. Topics ranged from the fact that SCD is globally neglected, awareness at every level with all stakeholders need to exponentially increase and that education, care and treatment access are tainted by stigma and structural inequities in all geographies. The panel debated commonalities with and learnings from programs that tackled HIV, Hepatitis and malaria, and the fact that this childhood NCD is not on any international healthcare priority list… We touched on the critical value of horizontally integrated health care, community involvement and patient activism and how the community must to reframe the unmet needs narrative - 50y of productive life lost; increase mortality among young adults and older despite advances in science that can inexpensively transform this statistics; and opportunities in this disease area to focus on effective primary, secondary and tertiary prevention and care. Resource mobilisation was debated, both regional and global, and how sustainable public and private partnerships are needed to change the course of care. Governments must be invested to ensure sustainability but there are great opportunities of simple steps that can be taken NOW. We did not derive solutions, but I left this meeting extremely #hopeful that we have started a journey together that will endure and change the lives of people with this neglected disease. Recently, I received an invitation that prompted deep reflection. I was honored to be invited to speak at Gilead’s inaugural Global Health Equity Summit and share our experiences from Asia in tackling global health equity.   While we celebrate advancements in HIV, liver disease, and oncology treatments, achieving true health equity remains a challenge in many parts of Asia. So, how do we tackle this issue?   Reflecting on what good has looked like for us in the past few years, I've identified three effective tools: 1.           Empathy: We must extend empathy to those living with the diseases as well as their families, transcending socioeconomic and cultural barriers. Empathy fosters trust between healthcare providers and patients, enhancing effective access to care. 2.           Expansion: Equity necessitates expansion of rapport with stakeholders, i.e., governments, community organizations, clinicians, or researchers. Through partnership and shared goals, we can bolster community-led initiatives, amplify the voices of underserved populations, and promote people-centered care. 3.           Expedition: Through clinical trials, research, and local data generation initiatives, we accelerate access for patients. Our commitment to scientific innovation ensures that more frontline healthcare providers are involved in the journey so that more patients can benefit from the innovative treatments.   While we inhabit the same planet, healthcare realities can vary greatly, and we need different tools in our toolbox to help solve the issues.    As the saying goes, a divided house cannot stand. Together, we deliver better healthcare and choices to the people and places that need it most. This marks the beginning of our steadfast commitment to serve as a Bridge to a Healthier World.

The beginning of this century marked a critical turning point for global health with the creation of initiatives like @gavi (2000), @GlobalFund (2002) and @PEPFAR (2003). This led to the expansion of treatment for HIV, TB, malaria and new vaccines in LMIC with remarkable success. The high burden of disease coupled with the deleterious social and economic effect they were having on communities necessitated quick wins. In response, governments enacted laws and policies that allowed global health initiatives to operate as vertical health programmes. Global health initiatives made life-saving interventions available in LMIC, but didn’t catalyze much improvement in the overall functioning of health systems. Currently, more than 50% of people in the world don’t have access to essential healthcare services. #GPW14. The problem is global health initiatives operate in siloes within the broader health system. They have parallel supply chain systems for their commodities. They create data systems for diseases. And community-targeted behavior change interventions narrowly focus on diseases. To address this, there is an urgent need to re-think, re-imagine and re-design global health initiatives to be system-focused. This goes beyond having a “health systems strengthening” objective. Systems thinking should guide agenda setting and strategic planning. Adopting a system-oriented model with emphasis on primary health care will improve performance for the disease of interest but also other components of the system, thus yielding better value for money. Most importantly, it will fast track progress towards #UHC.

The National Institutes of Health (NIH) is committed to working with partners to provide solutions for those suffering with Long COVID. To this end, NIH is investing an additional $515 million into the Researching COVID to Enhance Recovery (RECOVER) Initiative to build on the work to understand Long COVID and inform treatments. These studies, and others will provide important insights into Long COVID and will improve our understanding of other infection-associated chronic conditions with similar symptoms to inform treatments.
