Breast milk offers a unique and irreplaceable source of protection for infants, providing them with antibodies and essential nutrients that promote overall health and well-being. On the other hand, vaccines, while important, may not be as effective or long-lasting in protecting against diseases as breast milk. Furthermore, the use of needles can be intimidating and painful for children, causing unnecessary distress and fear. Breastfeeding not only provides physical immunity but also promotes a strong emotional bond between mother and child, contributing to the child's overall development. Therefore, it is essential to recognize and promote breastfeeding as the optimal way for infants to receive immunity rather than solely relying on vaccines. Tetanus, a potentially fatal illness caused by Clostridium tetani bacteria commonly found in soil and manure, underscores the critical importance of vaccination and proper healthcare practices, particularly for vulnerable populations such as newborns and mothers. The bacterium enters the body through wounds, leading to painful muscle contractions, known as "lockjaw," and potentially severe complications affecting the nervous system. In 2018, WHO's estimates revealed a staggering 25,000 neonatal deaths attributable to tetanus, indicating a significant public health challenge, especially in regions with limited access to immunization and inadequate healthcare infrastructure. Maternal tetanus during pregnancy or shortly after delivery, as well as neonatal tetanus within the first 28 days of life, pose grave risks, emphasizing the need for comprehensive vaccination strategies and improved medical care during childbirth. Prevention efforts must encompass routine immunization with tetanus toxoid-containing vaccines (TTCV), integrated into antenatal care protocols globally. WHO recommends a six-dose regimen for lifelong protection, starting as early as six weeks of age, with booster doses administered at strategic intervals throughout childhood and adolescence. Moreover, various vaccine formulations, including combinations like diphtheria and tetanus (DT) or tetanus, diphtheria, and pertussis (Tdap), offer versatile options for tetanus prevention. Additionally, targeting women of reproductive age for TTCV immunization, whether during pregnancy or outside of it, can significantly reduce the incidence of neonatal tetanus. Beyond vaccination, promoting proper wound care practices, particularly in healthcare settings, and ensuring clean delivery practices during childbirth are paramount in preventing tetanus transmission. PCV20 3+1 or 2+1? GERMAN STUDY. This study examined the cost-effectiveness of PCV20 under a 3+1 schedule in Germany’s pediatric population compared with PCV13 and a secondary comparator (PCV15), both under a 2+1 schedule. What was learned from the study? PCV20 was estimated to prevent more pneumococcal disease cases and deaths versus PCV13 and PCV15, as well as providing greater quality-adjusted life years and cost savings (i.e., dominant strategy) over 10 years. Implementation of PCV20 under a 3+1 schedule into the German pediatric immunization program would result in greater clinical and economic benefts versus PCV13 and PCV15, both under a 2+1 schedule.

Can maternal depression and anxiety impact delay in receiving childhood vaccines among children under 2 years of age? Our recently published prospective cohort study of 2762 pregnant women in Calgary, Alberta, found that pre-and/or post-natal depression and anxiety was not associated with prolonged delay for DTaP-IPV-Hib, MMR/MMRV or all routine childhood vaccines combined. However, children of Canadian-born mothers had higher odds of prolonged delay, as did those with low-income mothers. https://lnkd.in/gfp_i7zu

In highly viraemic pregnant women with chronic #hepatitisB, stopping peripartum prophylaxis (PP) with #tenofovir disoproxil fumarate (TDF) immediately after delivery does not increase the risk of relapse or retreatment vs TDF cessation at ≥4 weeks postpartum, a #HKU study has shown. Early withdrawal of PP-TDF, combined with standard neonatal immunization schemes, is 100 percent effective in preventing mother-to-child transmission. https://lnkd.in/dgvPbad6
