We are pleased to share that St. Mary's and UnitedHealthcare have reached a new multi-year agreement! Patients with UHC commercial or Medicare Advantage products once again have in-network access to St. Mary's Hospital and Home Health Care/Hospice Services, as well as our outpatient facilities and services. Thank you for your patience and support as we worked to reach a fair agreement with UHC. You may once again schedule appointments and seek care at St. Mary's as normal. (Quick additional note: St. Mary's Medical Group, Sacred Heart Hospital and Good Samaritan Hospital were not affected by these negotiations and did not go out-of-network.) Important new brief out of the Bipartisan Policy Center: "The Future of Acute Hospital Care at Home" To ensure beneficiaries can continue to receive various levels of care in their preferred settings, the BPC brief provides immediate-term federal policy reform recommendations for the potential reauthorization of the Acute Hospital Care at Home model, including extending the waiver for five years, while outlining key issues for policymakers to consider as further data emerges. Recommendations largely align with: --Facilitating appropriate uptake and participation in the model; --Gathering evidence on the model to support high value, equitable, and comprehensive care; and --Ensuring program integrity and patient and caregiver safety. BPC spoke with a number of stakeholders in preparation for this brief, including several Coalition members. Review and share with your network: https://lnkd.in/gGsxJ22A #HospitalAtHome #HomeHealth

Rural Health Clinics need remote care options to serve the patients who rely on them. That's where Chronic Care Management came in for Kiowa County Hospital District in Eads, Colorado. By starting a #CCM program, KCHD was able to give their rural patients a 24/7 nurse line and 20 minutes of preventative care every month. Want to learn more about what CCM can do for rural patients? Well we’ve done it again! Today has seen the publication of the latest Adult Inpatient Survey results by the Care Quality Commission – and they certainly make fantastic reading for all of us at RJAH. We’ve got rather used to saying that over the last few years, but that doesn’t mean we should take it for granted, and we want to thank all of our people for their ongoing hard work and commitment. A total of 131 Trusts took part in the 2023 survey, which was taken (as usual) in November last year. During that month, 1,250 of our patients were invited to complete the survey and 856 did so – a response rate of 69% which was the best in the country, so a huge thank you to our patients for that as well. We are one of just nine Trusts categorised as achieving “much better than expected” results. Highlights to note include: 🍽 Our food has been rated No 1 in the country for the 18th time in the last 19 years. We almost take this for granted now – we shouldn’t; it is a phenomenal achievement. ✔ We rated No 1, for the fourth year running, for patients reported that their room or ward was clean. 🤝 We score No 1 in the country in terms of involving both our patients and their family or carers in shared decisions about their treatment and care.

New data published by NHS England reveals the overall referral to #treatment waiting list, which is currently at a record high of 7.8 million, is made up of 6.5 million individual #patients meaning 1.3 million are patients on multiple #RTT pathways. Stella Vig, NHSE’s clinical director for secondary care, told HSJ around 2 to 3 per cent of the individual patients on the waiting list are on four to five #pathways or more, equating to around 700,000 cases. She said: “There are real opportunities to use this #data to help develop personalised care plans, merge #appointments, or have multiple appointments on the same day. As well as being better clinically and more efficient, it will be far more convenient for patients and help reduce missed appointments.” This could be a stretch – the #NHS finds it difficult enough to manage the existing waiting lists, so how would it cope co-ordinating multiple pathways? Insource Ltd’s Patient Pathway Plus (PP+) can help. It enables #healthcare organisations to confidently manage all #elective care pathways, by automatically linking and checking all pathways against all applicable rules and creating and managing workflows for the teams who administer them. And ensures no patients are ‘lost’ in the system. Today, home health and hospice providers are facing several operational challenges with on-call staff, including staffing shortages, limited budgets, and the need to improve patient outcomes as a part of the shift toward value-based care. Fortunately, centralized triage has become an essential backbone for home health and hospice providers to rely on. It creates the perfect solution for reducing the burden on caregivers by providing clinical overflow support, and more compassionate and efficient approaches to home-based care. https://lnkd.in/gxc8KMrU

This was crucial to me during my hospital stay as a very young adult, (not knowing I would be in this health arena later in life) and during my adult years of hospitalization. A team of experts collorbating in my care was necessary in my recovery process and understanding next steps. Very proud to represent and be a part of Patient Guide Solutions and a patient's journey. #healthcare #patientexperience #patientoutcomes
Adopting a multidisciplinary team model enhances communication between healthcare professionals and social workers, streamlining coordination of care plans prior to patient discharge. Effective discharge planning is crucial for reducing hospital readmissions and boosting patient outcomes. To optimize your discharge processes, focus on enhancing team communication, managing medications, engaging caregivers, providing targeted education, and ensuring thorough post-discharge follow-up. Implementing a proactive discharge strategy not only alleviates financial pressures on your hospital but also boosts patient satisfaction and overall health. What are you doing to help patients understand their care plan before discharge? #PatientGuideSolutions #PatientExperience #PatientCommunication

Recent data from WellSky revealed that 25-30% of inpatient hospital stays are avoidable. When 40% of readmitted patients say that challenges at home, like transportation barriers or a lack of social support, contributed to their readmissions, the importance of companion care cannot be overstated. That’s where #Papa comes in. Our own data shows that members who have access to in-home social support demonstrate more informed use of healthcare services. These members often opt for lower-cost care and recovery at home over inpatient or facilities-based services. This shift in behavior has contributed to a 9% reduction in medical costs among Medicare Advantage members and even greater reductions for those previously identified as socially isolated. #PapaPals, like Jean, play a vital role in addressing members’ unmet needs. With personalized help right to the front door, Papa Pals ensure members receive the attention and support they need to prevent unnecessary hospital stays, support healthy recoveries, and improve overall well-being. The impact for members, and their health plans, cannot be overstated. As we reach the end of the first half of 2024, how confident are you that you're moving the needle on costs this year? The recent coding changes make it more important than ever that value-based care organizations focus on reducing costs. Advance care planning (ACP) offers significant potential for cost reduction and improved patient experience. The evidence for ACP is compelling: • Studies show that ACP can reduce unnecessary hospitalizations by up to 30% for patients near the end of life. • ACP improves the quality of life and end-of-life care by increasing the utilization of palliative and hospice services. • Patients who engage in ACP are more likely to receive care aligned with their preferences. • ACP reduces stress, anxiety, and depression for families after a loved one's death. ACP programs can be a true win-win - lowering costs while significantly boosting patient and family satisfaction. So, I'm curious: How are you incorporating advance care planning into your value-based care strategies? Let's discuss how we can leverage ACP to improve outcomes, enhance patient experience, and drive down costs in a meaningful way. #ValueBasedCare #AdvanceCarePlanning #PatientExperience #ACO
