Patients with advanced and highly astigmatic keratoconus who don’t desire or do not tolerare rigid contact lenses might benefit from intrastromal corneal ring implants. The following patient is part of a bigger group which would otherwise need corneal transplantation. Surgical technique was manual, since public hospitals usually do not provide femtosecond laser in Brazil. The patients become amazed of being able to wear glasses or soft disposable lenses again after surgery. Post-op refraction is usually less astigmatic than axial curvature. Patients’ quality of life are highly improved. The segments are calculated based on the Ferrara Nomogram and are inserted between 5 and 6 mm.
