I say no to more burdens placed on delivery team members, especially without carefully thought out and tested policies - that we have lacked for 40 years of designs. Wanting to help with housing and food is natural. But 1. What happens to personnel budgets when more funds are diverted to do SDoH 2. Who does the work 3. How does this impact their efforts 4. What are the costs? 5. What ongoing burdens are required to keep up and arrange resources? 6. Is this going to help practices already behind with fewer and lesser delivery team members? First of all, health care delivery must be able to actually deliver the care and caring. So first we must have a financial design that completely funds most and best delivery team members for at least 95% of our population in primary care, mental health, women's health, basic surgical, and geriatrics. Currently about half of the population suffers from half enough of each of the basics as well as fewer and lesser delivery team members - the ones most important in integration, coordination, preparation for higher functions, maintenance of contacts, and close work with patients and caregivers. We must also commit to add 70 - 90 billion to each of these basics plus coverage of the costs of any new machinations involving obsessive measurement disorder And we have to identify some 200 - 300 billion in targeted cuts to those doing well because of budget neutrality Another challenge is slipping this by regulatory capture dominated by non-basic people But if we force social determinant burdens upon fewer and lesser delivery team members across vast regions of our nation, we can only expect worsening turnover losses and costs, an even less experienced workforce and delivery team members, more burnout, and worse.

Great kick off for HIMSS Global Health Conference & Exhibition with the Virtual Care Forum - Scaling Virtual Care. Camille Bradley made an excellent point to look at async care as patients just have a lot to do in their life and that there is in fact a natural conflict of interest when it comes to time (patient vs provider) If we want to be truly patient centric we need to rethink and adjust processes to meet them at the time and the way they need care. #virtualhealth #virtualcare #digitalcare
