One of the beautiful aspects of a Self-Managed model is its "Community ❤️ Focused" approach. 👨👨👦👦 This means that each team is dedicated to providing in-home services with a primary focus on their local communities. ✔️ Our care teams are not only experts in their fields but also deeply knowledgeable about their local areas, fostering strong relationships with the people they serve. ✔️ By building connections with families, schools, physicians, hospitals, nursing facilities, and more, our care teams and community liaisons ensure that when someone needs in-home services, we can immediately step in and provide the quality care required. Focusing on the community: ⏳ Reduces the time to admission. 🙌 Improves the quality of care. ❤️ Results in much better outcomes. If you’re interested in learning more about our community-focused approach, please reach out to us. CC: Assaf Shalvi Vivek Nanda

Unmet social needs can have a negative impact on an individual's overall well-being, resulting in a sense of emptiness that goes beyond just bodily necessities. Feelings of isolation and emotional pain can arise when individuals lack meaningful relationships, understanding, and a sense of belonging. The prioritization of unaddressed social needs plays a pivotal role in cultivating a nurturing community and advancing mental well-being, hence bolstering the whole social structure. The evaluation of unmet social needs holds great importance for mental health professionals, as it provides essential understanding of the overall well-being of individuals. Mental health experts have the ability to customize interventions to address unique needs by recognizing deficiencies in social ties, support networks, and a feeling of community. The acknowledgement and resolution of unfulfilled social requirements not only enhance therapeutic approaches but also empower individuals to cultivate resilience and bolster their mental well-being in the face of isolation and disengagement.

Something to consider…have you screened your own hospital employees for unmet social needs? Imagine you are facilitating social needs for patients day after day, month after month. All the while, you yourself are struggling with food insecurity, housing insecurity, domestic violence, etc. I cannot fathom the moral injury we place upon our workers who are expected to serve others, while not receiving the same attention who may be in tremendous need. We must do better for our patients, our care teams, and our communities. We talk about #moralinjury. We talk about our patients unmet social needs. But we CANNOT forget that in order to best serve our patients, we MUST assess our teams who are expected to support the needs of others. When did YOUR hospital last assess your employees’ social needs? And did you provide that assessment in a safe and anonymous way? And then provide access to services? Even in the early stage discussions of increasing user pay in aged care, the national conversation appears to be already having an impact. Despite not being our target audience at all, Community Home Australia is already receiving enquiries for full fee for service admissions (our first user pay guest will be moving in soon) and we have had 5 new private pay members join our programs in the last 3 days. While the sector is heartily applauding the introduction of more user pay to fill the coffers, are they actually inadvertently starting the great movement away from institutional care? There is not a nursing home in the country that can match us for staffing ratios, educational standards, community connectedness, meaningful engagement or person centred care. Clearly people don't mind paying, but expect something more than mediocrity and institutionalisation. 🌟 Did you know that more than one in 10 California nursing facility residents have low care needs and could potentially thrive in more residential, supportive settings like assisted living communities? 🌟 The California Health Care Foundation's latest publication, "What is Assisted Living? Opportunities to Advance Community-Based Care for Medi-Cal Enrollees," highlights a critical opportunity to enhance care for Medi-Cal enrollees by offering them the option to live in settings that align with their needs and preferences. Programs of All-Inclusive Care for the Elderly (PACE) play a pivotal role in this landscape. PACE is designed to serve individuals at risk of nursing home placement, providing them with the comprehensive care they need while allowing them to remain in their own homes and communities. This includes those residing in assisted living facilities, where PACE can support their desire for independence and a higher quality of life. Thank you to the California Health Care Foundation for highlighting solutions that honor older adults' wishes to live with dignity in the community. #ElderCare #PACE #AssistedLiving #CommunityCare #HealthCareInnovation #SeniorCare
