Navigating the Power Law in Big Pharma: The Case of Keytruda and Industry Dependence on Blockbusters In 2018, while working at Merck, I observed firsthand the immense financial impact of Keytruda (pembrolizumab), a monoclonal antibody targeting PD-1. This revolutionary drug not only transformed cancer treatment by enabling the immune system to fight cancer more effectively but also generated a staggering $7.2 billion annually, accounting for 17% of Merck’s revenue in that year. Despite Keytruda's success, there was a palpable anxiety among senior leadership about Merck’s future sustainability. The reliance on Keytruda and a few other products highlighted a significant risk — with 38% of the company's revenue coming from just three drugs, any market shift could have dramatic repercussions. By 2022, Keytruda's contribution had grown to represent 36% of Merck’s annual revenue, underscoring an increasing dependency. This pattern is not unique to Merck; AbbVie's Humira and BMS's Revlimid have shown similar trends, dominating their respective company revenues. These observations raise crucial questions about the sustainability of current business models in pharma, where a handful of products often determine the financial health of a company. The expiration of patents and the introduction of biosimilars add further complexity to this dynamic. As we look to the future, how can companies mitigate the risks associated with such dependence? Diversifying product portfolios and investing in continuous innovation are imperative to maintaining a healthy, resilient business.
