Curious… Based on a recent post, a reference was made to a physician possibly being a “quack” because he offered a diagnosis and treatment plan that was unconventional and not considered “standard of care.” The dictionary definition of a “quack doctor” is: an unqualified person who claims medical knowledge or other skills. How did we stretch this meaning from “unqualified and without the knowledge or skills he claims to have” to now meaning “any physician who doesn’t practice under the generally-accepted standard of care?” While the former definition seems logical to me, the latter creates a culture that treats medicine as a groupthink rather than an art. And aren’t we, as recipients of care, whether quack or not, allowed to make individual decisions about the suggested treatments? Personal example: I visited a functional medicine physician (MD) to identify areas I could optimize. No symptoms, no concerns. Lab testing showed I had H. Pylori, a common bacteria affecting the GI tract. Conventional medicine would have treated me with a combination of antibiotics and PPIs. She treated me with a combination of supplements. My PCP later told me that was a bit crazy, but he agreed to retest. H. Pylori - negative. Was the functional medicine doctor a “quack?” I’m sure my PCP thought so. Regardless of labels, her treatment plan stepped away from conventional medicine and was both effective and safe. So I’m wondering, what is the currently accepted societal definition of a “quack?”

Finding macular degeneration early and treating it immediately with the most powerful therapies available is critical so we may do our job and preserve our patient’s vision. This goes well beyond the question of which supplements to prescribe. Only talking about which supplements to prescribe is like debating about what color squirt gun is best at putting out a forest fire. Hormones matter. Methylation biochemistry matters. Inflammation matters. Obviously nutrition and genetics matter too. And lifestyle choices as well. Let’s upgrade, modernize, and advance the way we approach the treatment of AMD. Let’s discuss it all and debate about what our standard of care shold include… #maculardegeneration #optometry

June 24th. - Update on post Seeing Clearly: The AMD Supplement Debate Hello Friends; can I ask that you take time to participate in the survey. It will take a couple of minutes. We need your input. https://lnkd.in/exHJubA2 June 21st 2024 - Update on post Seeing Clearly: The AMD Supplement Debate Ok, it is now time to start this important research. We are live. We look forward to hearing what you have to say. Thank you for your time. https://lnkd.in/exHJubA2 Tuesday June 4th 2024 - Update on post Seeing Clearly: The AMD Supplement Debate Dear friends, Just for update. Firstly, thank you all for the overwhelming/positive response. There is clearly a need/appetite for this discussion, and for/from all stakeholders. Over the last few days, I have worked with my research colleague Dr Anne Graham Cagney who is a leading scientific specialist in Qualitative research. This type of research is suitable to collect and interpret survey, interview, and seminar data. This is exactly what we are going to do. The Idea is that the research will be conducted in line with good scientific practice and with a view to publishing in a way that will inform all stakeholders and provide a reference for positive change. What happens next? This week, Anne Graham Cagney PhD and I will publish the plan/protocol for this research, but please be assured; the research question(s) will be your questions. We will kick off with an online seminar and the research questions will be developed as part of this event. Thank you for your support, patience and participation. This will be the first of its kind. Back soon, Prof Nolan. Saturday June 1st 2024. Dear Friends. Can I ask for a proper debate. Please. Firstly, I must say that my respect for the medical profession is total. I am so greatful that I and my family can get get care whenever needed. Some of my best friends are medical doctors. Amazing people. Now to my point. Daily I receive a comment/query regarding supplements for AMD. Which one, when, what age, why meso-zeaxanthin?, AREDS, AREDS2, stability, how to measure, and so on. The idea that supplements and macular pigment are only useful at and for a certain stage of disease is irrational. The ophthalmology medical default to AREDS2 ONLY for a certain stage of disease is misinformed and misinforming. AREDS2 formula used in study is very different to the supplement being sold to patients! So here’s is what I propose. A controlled debate between doctors, scientists, patients and industry. All welcome. If you can share and we get to 1000 thumbs up I will arrange a live event via the NRCI. There is no point in me speaking to myself. Sometimes I feel like I am. This is “for the people”. Please help me develop this research idea. It could be very cool. Thank you for your time. Prof Nolan. #bonconference #AMD #AREDS #AREDS2 #supplements #vision

CAN WE GET SERIOUS NOW? This debate is long over due when: -AREDS 2 science is 20 years old, - Lutein and Zeaxanthin are sourced from different provider by leadings AREDS2 formula. - Meso-Zeaxanthin, THE CRUCIAL CAROTENOID, is not in the study - 70% of ocular carotenoids products fail to meet their label claims - Companies telling you to avoid MZ have traces to notable amount of it in their supplements So yes! FOR THE PATIENT SAKE, Let’s have that debate!
