A few years ago I worked for a mining company and was on call when we had a section of rib/roof fall and strike two miners working in the area. The accident was pretty serious but thankfully both miners were able to recover, due in large part to an active mine rescue team that was trained and prepared. As the investigation unfolded we found at that when new miners had come to the area they had recognized the hazard but no one said anything or at least nothing was done to mitigate the hazard until the two people were injured.
