According to a study at the Journal of Palliative Medicine, home-based #PalliativeCare, delivered to people not yet eligible for hospice, reduces hospitalizations, increases length of life, and extends the continuum of care delivered at home for people with serious illnesses and their #caregivers. "Live discharges are disruptive to patients and families and could mean that terminally ill individuals do not receive adequate care at the end of life, according to the two authors, who argue that these incidents are critical care transitions that contribute to increased hospitalizations and emergency department visits.“ Although a discharge due to stabilization or lack of decline might sound like a positive outcome — and is often presented as such by hospice teams — most individuals are left struggling to replicate the supportive services that hospice provided — medication delivery, supplies and equipment, access to support around the clock, and routine visits with interprofessional team members,” the authors wrote. “When these services cannot be replicated, patients and caregivers are left to deal with the loss of these services, citing experiences of grief and abandonment.'" #hospicecare #medicare #endoflifeadvocacy

Providing quality palliative care is more critical than ever. The increasing number of hospitalisations for individuals receiving palliative care highlights the need for enhanced education and skills among aged care staff. As Australia's aging population grows, the demand for palliative care will double by 2050, underscoring the urgency for proactive planning. Camilla Rowland, CEO of Palliative Care Australia, emphasises the importance of enabling older individuals to spend their final days in familiar environments, including residential care settings. Despite challenges, several aged care providers are embracing palliative care, with training programs like ELDAC gaining traction. However, out of Australia's 2800 aged care facilities, only around 110 have fully integrated palliative care into their practices. To address this gap, there are opportunities for aged care staff to upskill in palliative care through programs like ELDAC and PEPA. 7moAs a member of The Coalition to Transform Advanced Care (C-TAC) I applaud this work and am encouraged by the outcomes. Personally, our family, in partnership with with my father’s PCP, did exactly this and also saw dramatic reductions in ED visits and hospital admissions … and that was just one patient…my dad. We honored his wishes to be cared for and stay at home. Eventually we had hospice step in and we were blessed to have kept him at home. His life was extended and he did not need as much acute care as prior was the case. The family caregiver role in all of this, caring at home, made the Palliative care team’s impact more effective. We need to talk about that more. The family caregiver is far more valuable than is currently being credited. My work documents that value and would love to talk with others who are interested in the model that aligns palliative care with the aide of family caregivers.
