According to a study at the Journal of Palliative Medicine, home-based #PalliativeCare, delivered to people not yet eligible for hospice, reduces hospitalizations, increases length of life, and extends the continuum of care delivered at home for people with serious illnesses and their #caregivers. As a member of The Coalition to Transform Advanced Care (C-TAC) I applaud this work and am encouraged by the outcomes. Personally, our family, in partnership with with my father’s PCP, did exactly this and also saw dramatic reductions in ED visits and hospital admissions … and that was just one patient…my dad. We honored his wishes to be cared for and stay at home. Eventually we had hospice step in and we were blessed to have kept him at home. His life was extended and he did not need as much acute care as prior was the case. The family caregiver role in all of this, caring at home, made the Palliative care team’s impact more effective. We need to talk about that more. The family caregiver is far more valuable than is currently being credited. My work documents that value and would love to talk with others who are interested in the model that aligns palliative care with the aide of family caregivers.
