To continue with my previous post where ASV was used to ventilate an obstructed patient: Do we care about how high is PIP when Pplat and DP are fine? Do we care about the cc/Kg IBW when DP is 12? Where is the best balance between providing an adequate alveolar ventilation while minimizing air trapping? Something to remark is that PEEP in ASV is selected manually by the operator: Does PEEP selection has any influence? Should PEEP be set to 0? Why yes or why not?

During the delivery of a breath, we can control only one variable at a time: Pressure or Volume. When we control P, the controller can target P to a constant intra-breath level or to an intra-breath level variable moment to moment. The constant intra-breath target P could be at the same level for all the successive breaths when it is decided by the operator: PC-AC, PS, APRV or it could be different when is automatically decided by the machine: PRVC, VS, ASV, IntelliVent-ASV. The variable intra-breath target P level is used by ATC/TC, NAVA and PAV. Since the controller follows and amplifies a variable signal, so the resulting P will be variable moment to moment.
