The impact of persistent low level viremia (PLLV) and viral blips on clinical outcomes is not always clear in persons living with HIV. In this real-world effectiveness study, PLLV and viral blips in the year prior to starting cabotegravir/rilpivirine (CAB/RPV) was associated with on-treatment viremia despite having an undetectable viral load at time of initiating CAB/RPV. https://buff.ly/3SqPkvD

It's easy to understand on the graphic how the Kynurenine pathway can cause HIV-Associated Mitochondrial Dysfunction (HIVAMD) and accelerated aging on PLWH on ART. There is something wrong with the Quinolinate Phosphyribosyl Transferase (QPRT) enzyme on PLWH otherwise most of the Quinolinic acid would transform to NAD(+) ("De-novo" pathway). I think chronic inflammatory states like HIV lean the pathway toward 3HK, 3HAA, and Quinolinic acid formation with LESS NAD(+) production. Like an intracellular pellagra state?. This webinar by CORE - Community Response to Reduce Inequalities, includes recommendations by the WHO to modernise testing algorithms to expand screening and for prompt linkage to care, and addresses Western Blot testing in the context of other effective and cheaper options to confirm an HIV diagnosis. WHO provides an overview on the diagnostics landscape for cost-effective testing in community settings. Participants will discuss ways in which to increase testing capacity of service providers. The needs assessment, conducted among CORE’s 14 community-based organisations across 12 European countries, revealed that testing kits used could be more cost-effective to save resources and scale up testing. Nelson Vergel, an HIV long-term survivor and friend of RORA Bio recently chronicled his journey as person living as an HIV-immune non-responder (“HIV-INR”). See the link below. People living as HIV-INRs face persistent immunological challenges in which their CD4 T-cell levels remain below 300 cells/mL (a better range is 500-1500). The low T-cell level makes them more susceptible to infection and certain cancers for life. RORA Bio is developing a first-in-class T-cell platform that may help support T-cell levels in HIV-INR patients should future clinical trials be successful. The platform consists of a newly discovered, long-lived, stem-like memory cell population that we call “RORA cells”. https://lnkd.in/eRRenhrY

The PURPOSE 1 trial demonstrated that after 52 weeks, no cisgender women acquired HIV infection after receiving subcutaneous twice-yearly lenacapavir.
