As a heat wave grips Arizona and the Southwest, surgeons at the Diane & Bruce Halle Arizona Burn Center are seeing an increase in elderly patients with severe burn injuries from contact with hot pavement and other surfaces. At a news conference this week, Dr. Kevin Foster, burn center director, was joined by 71-year-old burn survivor Robert Woolley of Phoenix, who last year suffered serious burns after falling on hot gravel in his backyard. The burn center is seeking to raise public awareness of burn risks associated with extreme heat. Story: https://lnkd.in/ggDNabna

Bessel in a very recent interview also talks of trauma as adhesive4identity as 'a dangerous thing' Somewhat similar, recently deceased Irish psychiatrist Ivor Browne spoke of trauma in terms of 'frozen present' So, what would solidify something in the present? Ivor Browne like Peter Fonagy talks of 'pseudo science'-'pre-mental equivalence' which goes back2neurologist Freud, his ideas on the constancy principle lens the 'repetition compulsion.' In even starker terminology, the cryptic Hegel in his purvey of the historical audit, within his vantage point of 18th/19th Century, 'history as a slaughter bench/slaughter of innocence. Around the same time, Johann Wolfgang von Goethe says 'In nature, we never see anything isolated, but everything in connection with something else which is before it, beside it, under it, and over it.' Looking at Ireland from Act of Union in the early 1800s/Catholic Emancipation and on to 1845-1922 (Ferriter, 2009 Occasions of Sin), apart from horror of Famine, refers2 'Devotional Revolution' (Emmet Larkin) as significantly underpinned by the radical fundamentalism of Vatican One in Rome, in combo with the Anglo-Irish Estates/French/Spain/Italian Clerical/Principalities&Estates, and, for Ireland, with signifiant USA help from Irish generations who had left our shores in prior centuries. Journalist Fintan O'Toole had made reference2 'lie of the land' (1998) a title of one of his books, and, in another book 'We do not know Ourselves' (2023) subsequently penned an article in 2024 Jan 2nd, referring to the overhang, which was before the very strong showing of the main parties in the recent local and EU elections, these parties whose lineage were directly immersed in the historical legacy of Ireland on into the evolving present. "Ireland has a history of keeping secrets that was not a genetic quirk – it was an institutional imposition."(O Toole 2024) Regarding this legacy, an award winning documentary trailer called 'Pray for our Sinners (2023) begins with this voiceover of a woman 'I just feel angry with the people who taught up this whole empire. "Historically, trauma studies have neglected vulnerable populations, as well as experience/aftermath of trauma in non-Western cultures. When you work with best physicians
Heather and Phil Freeman didn't know what to expect when their son Riggs was born with Pierre Robin syndrome and a cleft palate. As first-time parents, they were initially anxious but found reassurance through the exceptional care of Dr. Michael Jaskolka and the multidisciplinary team at Novant Health New Hanover Regional Medical Center. Riggs underwent three surgeries before his first birthday and is now a thriving three-year-old. Watch the Freeman family's full story here: https://lnkd.in/e-b3J66S

Delighted that our work, lead by my colleague Jared Wohlgemut, on the accuracy of pre-hospital clinicians to detect major haemorrhage has been published in TSACO. I think we tend to assume that experienced clinicians have excellent diagnostic skills, even in difficult situations. This study, along with our previously published work on the detection of life and limb-threatening injuries (https://lnkd.in/eR4YjRqY), quantifies just how good (or indeed, not so good) we are at this. Only 70% of major haemorrhage cases, defined by receiving at least 3 units of PRBCs in under 60 mins within the first day of admission, were detected by pre-hospital clinicians - penetrating injury and abdominal trauma made this more likely to be missed. Whilst we should commend the 94% specificity of clinicians, ensuring minimal overtriage and therefore limiting trauma team fatigue, the delay in recognising major haemorrhage has a significant impact on patient outcomes - in this study we found that non-detection of major haemorrhage leads to a 3-times increase in mortality. Recognising the limitations of our diagnostic accuracy as clinicians is key to being able to understand how to support injury identification and decision-making in trauma to improve outcomes for our patients.

“Just get LASIK when you’re older…” “It cures your short-sightedness”. Not true! Though it correct the vision and not needing to rely on glasses or contacts anymore, the fact is that the eyeball has physically grown in length! This is not reversible. Moreso, it makes those of us who have myopia more prone to getting other eye conditions such a detached/torn retina, earlier cataracts or glaucoma, as mentioned in this article. Do you know anyone who has or had both cataracts and glaucoma? Share in the comments
