We cannot have equity in health care without a Physician Fee Schedule that prioritizes payment stability and predictability for office-based specialists and fundamental reform of the PFS to ensure the viability of office-based specialists. Chronic underfunding of the overall PFS, faulty analyses of specialty overcompensation and PFS rebalancing policies of reimbursement away from specialty care are widening health equity gaps in America.

I read several articles recently about the inclusion of patient activation measure (PAM) surveys into innovative approaches to redesigning healthcare. I was both hopeful and sad. Hopeful for the attention patient activation is getting to improve healthcare delivery but sad because I understand the reality of what surveys in healthcare become. Too often we transform administratively burdensome tasks like surveys into mere items to check off our list, overlooking the chance to use the data we gather in a meaningful, real-time manner. So to help keep PAM surveying as a meaningful care task, here’s a straightforward strategy for effectively utilizing PAM data to enhance patient care. Additionally, PAM surveys also create a foundation to address health disparities. However, one thing I’ve learned in the last 6 months - in our efforts to incorporate health equity, we tend to overcomplicate things. The process of screening for Social Determinants of Health (SDOH) frequently becomes an elaborate endeavor, burdened with extensive planning and slow initiation.
