At Premier Inc., my population health colleagues and I spend a lot of time speaking with hospitals and health systems about how best to move towards value-based care in a successful and sustainable way. For many organizations, that is a challenging proposition for the short-term. Investing in value-based care improves patient health and promotes cost-savings. Ochsner Health instituted value-based, remote patient managing programs to help members living with diabetes, high blood pressure, and other chronic conditions. The programs have done such an excellent job keeping patients healthy that Ochsner has seen a 3-to-1 return on investment. “Value-based care continues to stand out as the clear solution, driving down costs, providing enhanced clinician support, improving patient experiences and outcomes, and rewarding patient-focused care,” Ochsner said in a recent report.
