If virtual MSK companies can reach multi-billion-dollar valuations based on comprehensive non-surgical treatment and avoidance of procedures, why can't surgeons? We can -- if the proper mechanisms are put into place. MSK VBC has centered around surgical episodes of care which only reward measures directly tied to the procedure (lower readmissions, less post-acute care, fewer complications). They say nothing about whether the decision for surgery was appropriate or evidence-based guidelines were followed. The answer is condition-specific VBC and/or global subcapitation for specialty care. Implementing these models represents a significant change in the mindset of surgeons, payers, and employers. Care pathways must be carefully designed. Collecting and reporting data is paramount. Collaboration and transparency are critical. My time in Durham last week at Duke Med's Health Equity and Value-Based Care Summit providing a tantalizing glimpse into the future of MSK VBC. Clinicians and hospitals need to start embracing clinical grade, FDA cleared, remote patient monitoring and other digital health innovation at an accelerated pace immediately to help address the growing shortage in cardiologists and to prevent the current ones from burnout! Equally important is that Centers for Medicare & Medicaid Services need to adjust reimbursement codes and rates to more appropriately reimburse clinicians, clinics, hospitals and monitoring companies on the RPM codes for specific vital signs and chronic condition monitoring and management. "Nearly half of all counties in the United States do not have a single practicing cardiologist, according to new data published in the Journal of the American College of Cardiology.[1] Counties with no cardiologists tend to be “rural and socioeconomically disadvantaged,” researchers noted, and associated with a heightened risk of cardiovascular disease. The team behind the analysis examined HealthLink Dimensions data, calculating that 46.3% of all counties in the United States—and a whopping 86.2% of rural counties—do not have any cardiologists. That 46.3% breaks down to approximately 22 million residents in 1,454 counties. For people who live in a county with at least one cardiologist, the average round-trip distance to the nearest cardiologist was 16.3 miles.
