Here’s another one that highlights the power of personalized intervention: An Innovista care manager (CM) checked up on a patient, concerned about their high blood sugar levels and lack of primary care physician (PCP) appointments. High A1C levels are associated with diabetes and other complications. Read this article this morning. The growing shortage of primary care providers is concerning for the aging population needing care. This is something we are strategizing around at Intermountain Health. Simplifying processes, moving care upstream, and empowering patients in their care are some of those efforts. One strategy not mentioned in this article is the impact #pharmacists can have to help offset this shortage. Already #ambulatorycare pharmacists are providing comprehensive medication management for patients with chronic conditions like diabetes, high blood pressure, asthma, COPD and more. Expanding availability to pharmacists, working collaboratively with physicians, will help put a dent in the primary care physician shortage. #ambcareisthebestcare
