It's refreshing to see a surgeon adapting their technique to better accommodate high BMI patients, showing a keen awareness of the limitations of the conventional "one size fits all" approach to commodity positioning product designs. This industry is notorious for copy and paste positioning device designs. In turn, supply chains see them as undifferentiated, seeing only price as the differentiator rather than function or efficacy. When innovative solutions designed to address these challenges are introduced to the market, they are often evaluated based on commodity pricing tied to "contract pricing" as they fall under a positioning category. This can hinder staff from exploring other options if they are not part of the contracted agreements, depriving them of the opportunity to conduct a comparative value analysis. Surgical positioning should not be determined by cost or superficial similarities, but rather by a thoughtful consideration of the unique needs of high BMI patients and the implementation of ergonomic processes that prioritize safe and effective positioning care. Commodity positioning devices and their inherently poor process guidance create these conditions. Positioning devices should be measured through the lens of physics, physiology, and processes, rather than perceived cost.

MARS2 results coming in indicating that surgery post chemotherapy for mesothelioma is being shown to be inferior to only chemotherapy. I do hope there are more studies with regard to this. patient's symptoms of Chest pain and breathlessness due to the trapped lung and the parietal pleural involvement is distressing. with more advanced treatments at hand, better technology and better ICU management maybe the coming years another study Will show another way out. this lady operated in December - before MARS 2 came into print, benefited from the surgery and living a more better life. long follow up will show the OS. #thoughts #mesothelioma #pleurectomy #thoracicsurgery #thoracic #thoracicsurgeon

In this article, MedPage Today reports on a study using longitudinal data from STS's Adult Cardiac Surgery Database, which shows that women were more vulnerable to poor long-term outcomes after postoperative atrial fibrillation following cardiac surgery. Investigators found that “although postoperative atrial fibrillation (poAF) was less likely to occur in women than in men after adjusting for patient and surgery-related risk factors...there was a significantly stronger association between poAF and subsequent mortality for women.” The findings, published in JAMA Network Open, call for a more personalized approach to postoperative care delivery. Roux-en-Y Biliary stenting in bile leak An interesting case of post-operative bile leak, managed by percutaneous and endoscopic intervention. 54yr gentleman had undergone cholecystitis for GB stones. Postop he developed bile leak. ERCP was tried, but due to complete transection of CHD, it was not possible to cannulate IHBR. So percutaneous access of right hepatic duct was done with micro puncture set. Wire was passed through CHD rent into collection. Using snare, this wire was caught from down with ERCP. Wire was brought outside from mouth. Then over the wire plastic stent was placed. Interventional cardiology is a medical specialty that diagnoses and treats heart and blood vessel conditions without performing actual heart surgery. Interventional cardiologists use small tubes called catheters, inserted in the patient’s body through a puncture in arteries in the wrist or groin region. Working with these small tubes, which they put through the blood vessels, allows interventional cardiologists to avoid open-heart surgeries. Yet they achieve results that can be life-saving, like when treating a heart attack. Interventional cardiologists treat patients with vascular (blood vessel) and coronary (heart) diseases, and structural heart disease.
