They say that an ounce of prevention is worth a pound of cure. In aggregate, that's true, and demonstrates very nicely the overall mission of public health. In some cases though, the value of prevention is so high relative to that of cure that, rather like dividing by zero, the ratio is incalculable. In such situations, even one case of illness represents a failure of the system. Congenital syphilis (CS), the infection of an infant by an improperly or untreated mother is one such case. Cooper and Sanchez, in Seminars in Perinatology, say that EVERY case of congenital syphilis is a failure of the public health system. That's because with adequate prenatal testing, up to three tests recommended for at-risk pregnant women, and adequate treatment--injections of bicillin, no infant should have to face the crippling and even deadly consequences of CS. Children born with CS face crippling physical deformity, developmental disability, palsy, blindness, and even death. While they can be treated and cured of syphilis, its effects are often lifelong. The inadequacy of prenatal care in the United States compared to other developed nations is well documented on many fronts. In the face of a rapidly swelling epidemic of syphilis among women aged 15-44 nationwide, however, it is specifically performing inadequately to prevent CS. Across the nation, cases rose 754% between 2012 and 2021, and continue to rise sharply, with 3,755 cases reported in 2022, or 102.5 per 100,000 live births. Among the most vulnerable populations, such as Alaskan Indian/American Indians, the number approaches a staggering 1 out of every 100 live births. Every one of those cases is somebody's precious child, and each case is ultimately preventable with a greater, awareness, effort, and investment in prenatal care. Easier said than done, I know, but we must start somewhere. Prevention is possible, can be relatively painless, and can work every time. It’s #pcosawareness month and one of the under-diagnosed symptoms/complications is hypo-lactation in nursing mums. Remember that an absence of diagnosis is not an absence of illness. In our practice, we have consulted with women who had PCOS but were unaware and the only symptom that stood out for them, was a very low breast milk supply. And this is not an issue that is fixed with increased breastfeeding/pumping. Hypo-lactation in #PCOS is thought to be due to insulin resistance that can sometimes occur, and also a decrease in breast tissue with milk glands.
