The proposal by AMREF CEO Dr. Githinji Gitahi to recalibrate the #UniversalHealthCoverage (#UHC) approach in African countries towards a more equity-focused model deserves thoughtful consideration. The argument hinges on pragmatic resource allocation in the context of financial constraints, a common scenario across the African continent. By directing limited resources towards the poorest and most vulnerable, UHC initiatives could achieve more immediate and impactful outcomes in health equity. This strategic prioritization aligns with the principle of 'Proportionate Universalism' which advocates for the intensity of support and intervention to be proportionate to the level of disadvantage. Data indicating the financial limitations of African countries to implement a UHC model akin to those in developed nations adds a layer of empirical support to this proposal. It suggests that a universal approach, while ideologically appealing, may not be feasible or sustainable in the current economic climate of these nations. Instead, a targeted UHC agenda would more effectively bridge the health disparity gap by focusing on those who lack the means to access healthcare services. From an organizational development and program management perspective, such a focused UHC agenda may also enable more precise monitoring and evaluation (M&E), facilitating iterative improvements and the efficient use of resources. By concentrating efforts where the need is greatest, it is possible to create a ripple effect of improved health outcomes that contribute to broader social justice and sustainable development goals. It's critical, however, to ensure that this focused approach does not inadvertently perpetuate inequity by excluding middle-income populations who, while not the poorest, still face significant barriers to accessing healthcare. UHC is a destination with a winding difficult path and must be achieved systematically and in context. For low resource settings where Goverment taxes are low and population high, a pro-poor approach is the first and important step followed by a focus on disease prevention. Sub-Saharan Africa affords only $40 per person per year of public expenditure on health services while Europe spends $4000 and US $5000 of public money. Its clear our paths can never be the same though the goal is the same - a pro-poor UHC as a starting point and a tighter focus on disease prevention is the pragmatic path.

Achieving UHC in Africa starts with deep policy shift

To achive universal Health Coverage we need to focus on equity and diverse Health services. There are communities left behind when it comes to access to health services due to stigma and discrimination, the nature of health services they require and the societal view. Have you ever taken time to think why is cancer on the rise despite the efforts made in vaccination and other preventive measures? There is one thing we are missing, male involvement, anal heath and anal cancer screening. Men also need to access HPV vaccination for us to achive our objectives in the first against cancer and achive equitable health. #ANALHEALTH, #GOOD HEALTH AND WELLBEING.
