🌀 WORKSHOP: Care Navigation/ Active Signposting 🌀 🔹Wednesday 24th April 2024 🔹9:30am - 12:30pm The key objective of Care Navigation and Active Signposting is to help patients access the right care at the earliest opportunity. It is a system of ‘triage’ carried out at the first point of contact with the GP Practice by non-clinical staff under direction of the clinical team. The key drivers for this patient centred approach is outlined in the 2023/24 contract. Patients must be offered and assessment of need, or signposted to an appropriate service, at first contact with the practice. Outcomes will include a more streamlined clinical workload, improving efficiencies in terms of access to services and increased patient satisfaction resulting in less complaints. It will achieve this by releasing valuable clinical time and resources. To ensure a seamless transition from the hospital to post-acute care settings, healthcare providers can consider implementing the following practical suggestions: Early Planning: Start discharge planning at admission to identify patient needs, post-discharge care requirements, and potential barriers to a smooth transition. Early planning facilitates timely arrangements for post-acute care services. This involves not only accurate screening of patients but also a thorough assessment to gather needed information to drive goals of care and later decision making. Interdisciplinary Collaboration: Utilize an interdisciplinary team approach involving physicians, nurses, case managers, social workers, and other relevant professionals. This ensures a comprehensive discharge plan that addresses all patient care aspects. Clear Communication: Ensure clear and open communication with patients and their families about the discharge process, post-discharge care instructions, medication management, and follow-up appointments. Use understandable language to increase comprehension. Be realistic about what can be provided and what can not. Ensure considerations are taken into the patient's service area, goals of care, and insurance benefits.
