    1. One of the most common beliefs about carbohydrates is that they promote weight gain. It is often thought that to lose weight it is necessary to reduce carbohydrate intake in general, but this is wrong to demonize carbohydrates as such. However, it is necessary to distinguish between simple sugars (which should be consumed in moderation) and complex sugars. In fact, there are no foods that make you gain or lose weight: all foods should come into your body in the right proportions, as part of a healthy and balanced diet. In foods such as pasta and bread, complex carbohydrates are found and: • are our main source of energy • contribute to a proper sense of satiety • contain fiber, which promotes intestinal transit and protects against colon cancer 2. Another common myth to be debunked is that carbohydrates consumed at dinner contribute to weight gain. It does not matter at what time of day you eat carbohydrates, but rather you need to pay attention to the amount of carbohydrates consumed daily and the total number of calories in your diet. This myth is based on the mistaken belief that if carbohydrates are consumed before bedtime, they are more likely to turn into fat in the absence of subsequent physical activity. In fact, energy consumption during sleep is not much different from any other sedentary activity that can be performed during the day. In addition, dinner, which also includes carbohydrates, promotes a night's rest by stimulating the production of serotonin, the happiness hormone. 3. Low-carbohydrate diet promotes weight loss: A balanced diet that helps to maintain health should provide enough carbohydrates, with a predominance of complex and whole carbohydrates. According to the major scientific nutrition societies, carbohydrates should comprise at least 50-55% of your calorie intake. There is no evidence that drastic carbohydrate restriction leads to any real benefits, including in terms of weight. Faster weight loss is observed only during the first period of such a diet, but later the benefits are no longer observed. Reducing carbohydrates inevitably means increasing fats and proteins, which, if taken in excess, can be harmful. Final confirmation comes from a 2018 study published in the prestigious medical journal Lancet: Low-carbohydrate diets increase the risk of mortality and cardiovascular disease.     Eczema is an inflammatory skin manifestation characterized by crusty redness and scaling and a rather prominent itchy component. Itching is the most bothersome symptom of eczema and can occur with different intensities (acute, sub-acute, chronic) depending on the underlying disease. In itself, eczema is not a serious condition, but it can create discomfort in both the social and occupational spheres. Types of eczema: • atopic dermatitis affects children aged 0-10 and is probably related to a genetic predisposition to develop allergies with itchy inflammatory areas typically located in the crook of the knees and elbows. It may manifest a reduction in symptoms (itchiness and skin dryness) in 80% of cases by the time of puberty, but may also persist into adulthood with a flare-up phase or blurred or absent disease; • irritative forms are due to, e.g., excessive hand washing or the use of chemicals or solvents that can cause our hands to lose the so-called hydrolipid film, which performs the protective barrier function; • allergic contact dermatitis are areas of skin inflammation created when they come into contact with a certain substance. Eczema is severely limiting for the patient because excessive itching leads to major problems in the social sphere, at work, and in the quality of sleep, so it must be managed with the utmost care. Usually, a clinical examination by the dermatology specialist, with an evaluation of the patient's medical history, is sufficient to diagnose eczema. In case of further investigation, useful ones may be: • allergy tests (e.g., patch tests) • skin biopsy (useful for assessing the presence or absence of more serious diseases) Eczemas, while very different from each other, almost all have the same treatment approach. These range from the daily use of emollient creams to replenish the hydrolipid film and epidermal barrier, which is our natural defense against eczema, to topical cortisone-based products where cream alone could not solve the problem. These products should be clearly chosen according to the patient's problems and characteristics. Other topicals include calcineurin inhibitors, which are now used very frequently when corticosteroid therapy has failed. There are also oral therapies that are used in particularly extensive forms of eczema, including cyclosporine, which achieves excellent results on major eczematous forms. On the front of therapies for atopic dermatitis, the so-called biologic medicines, already used for the treatment of psoriasis (interleukin-4 and interleukin-13 inhibitors), have made their entry a few years ago, and these will allow, in the not-too-distant future, almost total control of the disease. An additional natural therapy for those suffering from atopic dermatitis is balneo-thalassotherapy, that is, seawater combined with sun exposure, which is able in the period when we frequent seaside resorts to manage the problem very well without using any medicines in combination.

    To achieve a golden and even tan, it is important to adopt a diet that stimulates it with foods that are rich in substances that can neutralize the production of free radicals in response to UV exposure, preventing skin aging. Fruits and vegetables, in particular, play a vital role in our bodies because they: • Provide water, vitamins, minerals • Protect the epidermis • Replenish what is lost through sweating The pigment that gives us a darker complexion in summer is melanin, which also protects us like a filter from the harmful effects of solar radiation. Unfortunately, there are no foods that contain melanin, but many foods contain its precursor: beta-carotene. This substance is found in yellow or reddish-orange fruits and vegetables such as tomatoes, watermelon, strawberries, peppers, peaches, carrots. In addition to promoting melanin production by skin cells, beta-carotene has numerous properties: • Strong antioxidant power, so it can also protect the skin from aging • Beneficial effects on the immune system and circulatory system • It is the precursor of vitamin A, which helps boost the natural defenses against ultraviolet rays and counteracts the formation of free radicals Essential for a lasting tan are: • Red-colored foods because they are rich in lycopene, an antioxidant substance that prevents skin aging and protects it from the sun • Vitamin C found in kiwi, mango, pineapple, and blueberries, which promotes the synthesis of components such as collagen that keep the skin toned and elastic • Vitamin E found in extra virgin olive oil and sunflower oil, which can fight cellular aging and stimulate epidermal regeneration • Polyunsaturated fatty acids (e.g., omega 3 & 6), which also promote proper melanin formation within melanocytes and facilitate their transport to the outer layers of the dermis due to the fluidity typical of their structure, and are contained in nuts, oil, flaxseed, milk, avocado, and small, preferably local fish such as anchovies, mackerel, sea bass, sea bream, cod. The expert's advice: “It is well known that fruit contains a lot of sugar. Thus, one should always be careful of excess and limit oneself to consuming about 3 servings per day, including extracts or smoothies.” To hydrate the skin, especially in summer, it is essential to take in at least 3 liters of water a day: 1.5 liters are introduced through the water we drink, and the remaining 1.5 liters through fruits and vegetables. This is because water helps maintain the elasticity of the epidermis and prevents dryness and flaking. In order to maintain our tan for a long time, we can drink at least 8 glasses a day, which will provide us with the proper fluid supply to the body and a good skin glow.     Traveler's thrombosis, in scientific terms, is a deep vein thrombosis (DVT) and consists of the formation of a blood clot (thrombus) that most frequently occurs in the venous system of the lower limbs, although it can form in any part of the body. This clot inside the veins can completely or partially occlude the blood vessels: • Thus creating an obstacle to normal blood circulation • Causing a consequent increase in pressure in the affected area with swelling and pain This is a very dangerous condition, because the thrombus, or a fragment of it, can detach from its original site, forming an embolus that, through the venous circulation, can reach the pulmonary circulation, obstructing it. In this case, we speak of pulmonary embolism. In the case of traveler's thrombosis, the major risk factor is represented by increased venous stasis due to: • Prolonged immobility of the limbs • To the inactivity of the muscle pump and, in particular, of the calf muscles Other factors that increase the risk of venous thrombosis during or after a long journey are: • Having a tumor • Suffer from a coagulation defect • Have recently undergone surgery or suffered trauma • Advanced age • Pregnancy • Obesity • Smoking Deep vein thrombosis in many cases can remain asymptomatic and show no signs of itself. In other cases, it can manifest itself with: • Swelling • Pain and tenderness in one leg, which may become red and warmer than the other • Calf cramps If you have to travel for long distances, more than 6-8 hours, it is useful to follow some indications, to be adopted before and/or during the journey, which can help prevent problems. Among these: • Move your legs even while sitting to help improve blood flow • Perform ankle flexions, extensions, and rotations to activate blood circulation • Choose an aisle seat if possible so that you are freer to get up every now and then and walk down the aisle • Wear graduated compression stockings below the knee to help blood flow to the heart • Avoid tight clothing around the groin and calves, preferring comfortable clothes • Drink plenty of water to maintain good hydration • Avoid alcoholic and/or caffeinated drinks before and during the flight These preventive suggestions are valid for everyone even in the absence of risk factors.     Eating a balanced and adequate breakfast offers many advantages and benefits to the body and the mind. A good breakfast, first, gives the fuel needed to make the brain work and therefore improves intellectual performance, memory, attention, concentration, and to 'get the body going'. It also serves to: • Regulate metabolism after prolonged fasting at night • Mark the hunger-satiety rhythm of subsequent meals, helping to keep weight under control On the contrary, those who skip it end up overindulging mid-morning and at lunch, eating more than necessary and favoring particular foods rich in sugars such as snacks and sweets. All this triggers a vicious circle of glycemic peaks and glycemic drops (that is, sudden increases and drops in blood sugar levels) which result in 'hunger attacks' that over time lead to weight gain. This is why breakfast can truly be considered the most important meal. Obviously, it must be balanced, even better if done at home, sitting at the table, to dedicate the necessary time to it, just as you do (or should do) with lunch and dinner. As for the caloric intake, first of all, it should provide an average of 20% of the calories of the day.     Baker's cyst, also called popliteal cyst, is an accumulation of synovial fluid in the popliteal cavity, which is the lower limb region behind the knee. Synovial fluid, under normal conditions, fills the joint cavities, protecting and lubricating the joint structures and reducing friction of the moving bone ends. Injuries or inflammation from the internal structures of the knee can increase the natural production of synovial fluid, which is deposited and accumulated where it finds an outlet, namely in the back of the joint capsule, the lining tissue of the joint itself. Baker's cyst, therefore, presents itself anatomically as a swelling behind the knee due to the accumulation of excess synovial fluid. The size of the Baker's cyst can vary depending on the amount of excess fluid produced by the inflammation. It can reach d, dimensions from a few millimeters up to a maximum of 1/2 centimeters. The main symptoms: • Posterior swelling, the knee has a visible swelling in the popliteal cavity • Feeling of kneading and heaviness in the knee • Pain, which can range from mild to severe, significantly if the cyst is enlarged • Stiffness and difficulty moving the knee, especially after periods of prolonged inactivity In particular cases and in the presence of significant swelling, Baker's cyst can lead to compression of the vascular-nervous structures of the popliteal cavity, causing more significant problems, up to and including gangrene or paralysis of the entire lower limb. Conditions that can cause Baker's Cysts: • Meniscal injuries • Osteoarthritis and joint wear • Cartilage damage • Ligament injuries Baker's cysts are diagnosed during a specialist visit, and this type of anamnesis rarely requires more in-depth instrumental tests to be identified since they are clearly visible to the naked eye. However, it is possible that during the orthopedic evaluation, more specific check-ups may be recommended to identify the real causes of the disorder.     Punctate keratitis is a pervasive eye disorder that occurs more often in summer. Punctate keratitis is a disorder that causes damage to the cells of the corneal epithelium (the membrane that covers the external part of the cornea): the sun's ultraviolet rays, reflected by surfaces in conditions of high illumination, damage the DNA of these cells. Young and old, no one is excluded. In particular, you must pay attention: • Those who go by boat or to the mountains for an excursion on the glaciers. • Who reads for a long time by the sea. Those affected feel the sensation of a foreign body in the eye, almost as if it were sand. It is important to remember that the symptoms are not simultaneous with exposure to the sun: the first symptoms, in fact, can appear even hours later. What to avoid? • Remain without protection and therefore sunglasses. • Do not read for too long, as this activity tends to reduce your blink rate, making you even more susceptible to exposure keratitis. Punctate keratitis, when it occurs, can be very dangerous: if it appears when you are alone on a boat or in the middle of an excursion on the glaciers, for example, you risk not being able to keep your eyes open, and at that point getting back inside becomes difficult. A patient suffering from punctate keratitis is easily recognizable. In fact, the specialist, if he observes his eyes with the aid of a slit lamp, detects the presence of countless grayish dots on his cornea. The most effective treatment combines artificial tears with cortisone. The latter should be taken 2/3 times a day for 2/3 days at most, in order to minimize the problem.     Thanks to Roche's contribution, a new professional figure has been created at the Multiple Sclerosis Center of the IRCCS San Raffaele Hospital. The Research Nurse is a professional who is continuously trained and updated and possesses multiple specific skills in the field of Multiple Sclerosis (MS) aimed at: • To the administration and monitoring of approved therapies • To the management and coordination of clinical studies and the data collected in their context Implementing the Research Nurse at our MS Center has helped ensure prompt care for patients who need to begin or continue their therapeutic path at our facility. Furthermore, the improvement of the services provided has allowed: • A reduction in waiting times for the start of treatments • The expansion of daily coverage of administrations, thus promoting the care of people with MS treated at the San Raffaele Hospital center MS is a chronic inflammatory disease that predominantly affects relatively young adults in their most productive years. Over time, this disease can be associated with the development of clinical disability and cognitive impairment, which can significantly impact the quality of life of patients and their families, reduce work and social activity, and cost the National Health System (NHS) and society. The San Raffaele MS Center, directed by Prof. Massimo Filippi, has always been a national and international reference for patients with MS for diagnosis and treatment. It currently offers a wide range of therapeutic options. The Center has also collaborated for years in the development of new treatment approaches, participating in or coordinating clinical trials with new experimental drugs. The success of the MS Center is supported by a multidisciplinary team that interacts in managing patients and their problems. The team is composed of doctors, nurses, psychologists, physiotherapists, administrative staff, and various professional figures who collaborate in caring for people with MS. To date, the Center has followed more than 3,500 patients with MS, treated with first- or second-line drugs administered either during outpatient visits or through infusions in the context of a Day Hospital. Recent years have witnessed an increasing availability of drugs for the treatment of MS, with the recent possibility of offering treatment also to patients with progressive forms of the disease. New drugs, which require administration by infusion and appropriate monitoring, are in the process of being approved.
