Our recent diabetes care event in the South West was a success with insightful presentations from Dr Greg Ong, endocrinologist for Diabetes WA, Diabetes Connect Team. The event highlighted the importance of technology in diabetes care, including continuous glucose monitors and pump therapy options. Dr. Ong provided an in-depth overview of how these technologies can help alleviate burnout and improve patient outcomes and the feedback from participants was overwhelmingly positive. The event also promoted our commissioned services for chronic conditions and diabetes referral services in the region, including Silverchain Group Health Navigator service, Oseca (GP down south Ltd) Integrated Chronic Disease Care - Living Healthy, Diabetes WA telehealth service, Diabetes WA Diabetes Connect service for country GPs and WA Country Health Service South West Diabetes Clinics. #DiabetesCare #SouthWest Marina Johns

We shape, strengthen and sustain a health system that works for people, to achieve better healthcare particularly for those at risk of poor health outcomes. We are responsible for planning, guiding and directing investment towards important primary healthcare services. We identify priorities, link services, and fund care that is locally accessible, by working together with GPs, service providers, government and its agencies, and communities. We do this as the operator of the state’s three Primary Health Networks, which form part of the Australian Government’s national PHN program. We fund a range of primary healthcare services that helps more people access the care they need closer to home. We are delivering better health, together.
