Good people doing great things in the Wheatbelt!   Pharmacist, Daniel O’Driscoll is now offering Home Medical Review services from the pharmacy located at our Northam clinic.   According to the Pharmaceutical Society of Australia there are approx. 250,000 hospital admissions each year due to medication-related problems.  They estimate that 50% of this harm may be preventable.   A medication review is typically beneficial for patients who:    - Have three or more chronic conditions - Are aged 65 or over - Have recently experienced changes to your medications - Have recently moved into a residential aged care facility or been discharged from hospital - Take medicines that require close monitoring of side effects such as opioids, insulin, anticoagulants, antibiotics, NSAIDS, psychotropic medicines, or anticholinergics - Have experienced symptoms suggestive of a drug reaction - Have difficulty understanding and following your medication regime   As part of his review Daniel will discuss with patients medication compliance, storage, and safety. He will also analyze medication related drug interactions and potential side effects from medication and make recommendations as to how to mitigate those issues.  A referral from a GP or specialist is required.    For more information Home Medical Review fact sheet is available here: https://lnkd.in/gsEe-jzK #wheatbeltnetwork #wheatbelthealth #wheatbeltwa

In December we completed a full rebranding of our Northam facility with new signs installed on the outside and throughout the inside of the clinic. On Tuesday afternoon Michael and Chris from AAP Media visited us in Northam to film some drone video footage of our new building signage so that we can update our website. What do you think of the new video and signage re-brand? A big thanks to AAP Media for their project management and design and Ascot Sign Co. for fabrication and installation! #wheatbelthealthnetwork #wheatbelt #wheatbelthealth
