Enabling patient success by connecting everyone along the patient journey along the continuum between home and hospital.
About us
We work to solve the fragmentation of healthcare. We are a team of highly experienced clinicians and technologists that have come together on a mission: SIMPLER and BETTER health care, through one digital platform that connects everyone to enable patient success. Why? Health care is already highly fragmented. New technologies that are being added fragment the health experience even further. This impacts the most patients with complex or chronic health issues, as gaps and overlaps are the main drivers of care dissatisfaction, poor health outcomes and skyrocketing health costs. How? Careteam's digital collaboration platform ensures that patients, families and health professionals manage health issues proactively and are on the same page. This improves the experience and outcomes of care while optimizing costs and use of resources. Careteam is a powerfully simple, cloud based and AI-enabled digital platform that enables care planning, communication and task management, and offers a set of tools that integrate the other technologies that people already use. Careteam enables clinicians to create care plans for patients with complex and chronic health issues that include instructions and recommended resources. The patient or their main caregiver can then share the care plans with who they see as their circle of care and Careteam enables the activation of the care plan into a to do action plan. Health organisations can monitor whether the plan is being actioned and proactively intervene to prevent deteriorations. Built by clinicians and patients and sold to health organisations focused on scaling innovation and accelerating their move to patient-centered care in the community, Careteam the only system that flexibly adapts to all situations and offers patients full control of who on their support network gets involved. "Become an expert in your diagnosis. It’s your responsibility to know more about your disease than the medical professionals treating you." - Michael J. Fox. These powerful words from the iconic actor, author, and advocate remind us of the importance of self-advocacy and #patientempowerment in healthcare. After publicly disclosing his Parkinson’s diagnosis in 1991, Michael J. Fox launched his Foundation in 2000, dedicating his career to advancing research and support for those living with the disease. His journey shows how knowledge and proactive engagement can transform the patient experience as living with a chronic condition requires that patients, families, and healthcare providers working together as a team. So: 💡Stay informed. 💡Ask questions. 💡Build your support network. Understanding their condition allows patients to ask better questions, make more informed decisions, and actively participate in their care alongside their healthcare team, advocating for themselves with confidence. Of course, not everyone is able to become an expert on their own condition, and the burden of knowledge should never fall solely on the patient. Support networks around the patient, family and friends should also be well informed so they can step up when it’s overwhelming or simply too much to handle. We can’t end this post without doing a shout out to Michael’s advocacy cause: * Parkinson’s disease affects over 10 million people worldwide, making it the second most common neurodegenerative disorder after Alzheimer’s. It primarily affects those over 60, but around 10% of cases occur in individuals under 50. * Parkinson’s impacts movement and can also cause non-motor symptoms like sleep disturbances, cognitive impairment, and mood disorders. Managing Parkinson’s well requires a team approach involving caregivers, healthcare professionals, and a supportive community. * The Michael J. Fox Foundation (MJFF) is the world’s largest nonprofit funder of Parkinson’s research, investing over $1.75 billion to find a cure and improve therapies. It accelerates drug development and launched initiatives like the Parkinson’s Progression Markers Initiative (PPMI) to identify biomarkers. * The foundation also engages patients through platforms like Fox Insight, providing valuable data for research. Through public awareness campaigns, advocacy, and a collaborative approach, MJFF has significantly advanced understanding, treatment, and support for people with Parkinson’s, fostering hope for a cure. We understand the challenges faced by Parkinson’s patients and their circle of care. We worked hard to design a platform specifically for these needs, connecting patients, families, and healthcare providers to enable seamless communication and collaboration, so everyone stays informed, care is well-coordinated and patients have the best possible life with their condition. #MichaelJFox #quotesthatinspire #ParkinsonsDisease #PatientEmpowerment #HealthcareInnovation

🌍 October 12 was World Hospice and Palliative Care Day honouring the compassionate care that supports individuals and families facing serious illness. This dedication to compassionate care continues this week at the McGill International Palliative Care Congress (MIPCC) #mipcc2024, October 15-18, 2024, sharing best practices about #palliativecare. Since 1976, this biennial gathering is organised by McGill University, the home of Dr. Balfour Mount who first coined the term “palliative care” to highlight the notion of cloaking or protecting those living with terminal illness, with attention to quality of life up until the moment of death. The MIPCC brings together over 1,500 healthcare professionals and palliative care experts from around the world to share the latest insights, advancements, and approaches in end-of-life care, covering everything from pain management to the deeper existential aspects of care. The conference aims to: * Examine the core values of care and compassion at local, regional, and global levels. * Define challenges and solutions in conducting palliative care research. * Formulate methods to build community engagement in support of quality palliative care. * Assess the latest developments in science, medicine, psychology, pharmacy, supportive care, and spirituality as they relate to end-of-life care. * Evaluate approaches for justice- and equity-oriented palliative care practices. * Identify international and Canadian initiatives to provide culturally competent care. * Explore the role of the arts and humanities in palliative care and their impact on the quality of life for patients, families, and caregivers. * Examine social and health policy development, community outreach, and mobilization. At Careteam Technologies, we're committed to enhancing collaboration between healthcare teams, patients, and families, ensuring palliative care is personalized, coordinated, and compassionate. Together, we can bring comfort and care when it’s needed most. Contact us to find out more about how palliative care delivery can be transformed for the 21st century, using technology to increase human connection and quality of life at the end of life. Our collaboration with Trillium Health Partners and the Mississauga OHT to innovate in palliative care delivery is ushering a new era. #mipcc2024 #HospiceCare #PalliativeCare #WorldHospiceDay #CompassionateCare

Today, we honor British-Canadian computer scientist Geoffrey Hinton’s remarkable achievement in receiving the The Nobel Prize Award 2024 Physics for his pioneering work in Artificial Intelligence (AI), after spending many decades pursuing work in a direction that most of the world had written off, but found a home at the University of Toronto. Hinton’s words resonate strongly: “In science, you can say things that seem crazy, but in the long run, they can turn out to be right. We can get really good evidence, and in the end, the community will come around.” While Hinton’s perseverance and visionary thinking have undoubtedly paved the way for significant advancements in AI, he also reminds us in his most recent interviews of our critical responsibility to ensure these technologies are used for good. As we explore the potential of AI, we must remain aware of the ethical implications and the potential risks involved. At Careteam, we take these lessons to heart as we work at the leading edge of digital health. Our goal is to redefine healthcare delivery through innovative solutions, but we understand that this journey comes with challenges, including overcoming the inertia of the status quo and the “it can’t be done” style of thinking, just because for decades people tried without significant progress. As we celebrate Hinton’s contributions, let’s also reflect on the importance of responsible innovation. The future of healthcare depends on our ability to harness AI thoughtfully and ethically. Here’s to forging a path that balances bold ideas with a commitment to doing good! We are committed to navigating these complexities thoughtfully, ensuring that our advancements enhance patient care while prioritizing trust and equity, following the ancient Hippocratic edict of “First, do no harm” and ensuring that clinical leaders can lead this burgeoning AI opportunity, instead of AI becoming a powerful tool of control and denial of service based on poorly designed and tested algorithms - our vision for the future is one where physicians and patients using AI to make health better. #Quotesthatinspire #NobelPrize #AI #DigitalHealth #Innovation #GeoffreyHinton

Curious what an experiment with the new Generative AI tools yields? This is the result of a couple of minutes setting up Google's NotebookLM to look through our published content on our company website. To be honest, we expected WAY less... the words that would come to mind is clumsy, robotic, generic. Instead ... mind blown with how good this is. Listen to the recording https://lnkd.in/gp6BsbEa and see for yourself - it SO impressive on so many levels. We gave it a 5 star ⭐⭐⭐⭐⭐ rating because of: ⭐ How fast we obtained the result ⭐ How little "prompt engineering" we had to do ⭐ The massive amount of information processed so effectively ⭐ How well it captured key themes we deeply care about: the challenges for patients and families to navigate healthcare, the deep effort we have done to make our platform easy to use etc. ⭐ The effective use of two different personas in a radio interview like dialogue, where one speaker would have become monotone and less interesting What do you think of this result? Any insights from your experiments? #ai #generativeai #innovation #breakthrough #notebookLM
