Notably, cervical neuroendocrine carcinomas (NECs) may be genetically closer to common cervical cancer subtypes than to extra-cervical SCNECs found in the lung and bladder. For early-stage disease, treatment typically involves surgery followed by adjuvant therapy, including radiation and/or chemotherapy. However, the recurrence of these cancers poses significant challenges, as there is no standardized treatment approach. 📊A recent study assessed PCSK2 expression in GI NETs, finding 50% expression in duodenal, 59% in pancreatic, and 40% in ileal NETs, with 0% in stomach and 25% in rectal NETs. Incorporating PCSK2 into a panel with ISL1, NKX2.2, CDX2, and SATB2 resulted in 87% sensitivity and 93% specificity for classifying ileal NETs. 🔥Just released🔥 The European Neuroendocrine Tumor Society (ENETS) has released its 2024 guidance paper on managing well-differentiated small intestine neuroendocrine tumours (#Si-NETs). This update addresses the rising incidence of Si-NETs, often diagnosed incidentally or at advanced stages with metastases. The guidance emphasises the importance of rigorous biochemical assessment, functional imaging, and multidisciplinary treatment approaches. New insights from mid-treatment response to 177-Lutetium Dotatate in Advanced NETs❗ A study of 51 patients treated with 177-Lutetium Dotatate revealed: 41% achieved partial response (PR), 45% stable disease (SD), and 6% progressive disease (PD). PR was notably achieved after the second cycle in 70.4% cases, with mid-treatment PR predicting continued success (OR 20.9, p = 0.002). ⚡New efficacy data from NETTER-2 Phase 3 trial on 177Lu-Dotatate published in The Lancet! 📊Snapshot: Between Jan 22, 2020, and Oct 13, 2022, 261 patients screened and 226 patients randomly assigned to 177Lu-Dotatate plus octreotide LAR (n=151) or control octreotide LAR (n=75). Median PFS was 8.5 months in the control group and 22.8 months in the 177Lu-Dotatate group (HR 0.276; p<0.0001). Adverse events occurred in 93% of 177Lu-Dotatate patients and 95% of control patients. No drug-related deaths occurred. 177Lu-Dotatate significantly extended PFS by 14 months in grade 2/3 gastroenteropancreatic neuroendocrine tumour patients and should be considered a new first-line standard of care. https://ow.ly/AwNS50ScNMT #MedEd
