Airway stenting is one of the most fulfilling IP procedures when done properly, not only because of its technical complexity but more importantly the relief it provides to a patient with airway stenosis. In this case a 57 year old male with esophageal cancer presented with worsening stridor and dyspnea. The CT chest showed a large esophageal mass causing critical compression and infiltration into the trachea. Stenosis length with 1cm margins above and below: 6cm Tracheal diameter- 18mm Diameter at critical stenosis- 6mm Procedure: Inspection with LMA reveals severe compression of proximal trachea. Converted to rigid to place a covered SEMS 18×60mm under direct vision using a 3.9mm slim scope. Instant relief of dyspnea, patient extubated on table. Procedure time: 40 mins. Top tip: 1. You can use a slim scope to visualize the stent deployment reducing the need for fluoroscopy. 2. Prepare for the worst, expect the best. We prepared laser, cautery, snares, etc expecting to debulk the tumor. Luckily it wasn't necessary in this case. Procedure proudly done at our IP unit at Avenue Healthcare, Nairobi.
