We recently released an investigation report into a bus collision with a concrete barrier on 13 November 2022. The bus mounted the barrier and rolled over to the left, coming to a rest against the side of a drainage channel. Several passengers, including two who were ejected during the bus rollover, sustained injuries requiring medical treatment. The bus sustained significant damage. The investigation determined the driver likely experienced several factors which reduced their visual acuity and contrast sensitivity in a low light environment with limited visual cues. This impairment prevented the driver from effectively identifying the road and concrete barrier ahead, leading to a loss of situation awareness, leading the bus to be steered onto a misaligned travel path. It was also identified that the driver may have been distracted by an onboard system speed warning as the bus approached the barrier, which led to them looking down at the bus’s dashboard. This distraction removed the opportunity for the driver to recognise the imminent collision and take evasive action. The barrier involved was designed with a sloped leading/front edge, resulting in the barrier acting as a ramp when impacted by the heavy vehicle. The bus became airborne before rolling over into the adjacent drainage channel. The barrier had limited markings and reflectors reducing its visibility to drivers. The investigation also identified that the operator was unaware that the driver had a medical condition and was taking prescribed medication. This oversight resulted in a missed opportunity for the operator to assess the potential operational risks of having a driver with a diagnosed ocular disease working in low-light conditions. Had the operator known the medical risks, they could have considered control measures such as restricting night driving.
