If your CAPA brings someone to their knees, it won't solve your problem. I'm sure you've attended a meeting where someone reports a deviation, and you can practically feel the tension in the room with your hands. Not very pleasant, but very familiar. However, if everyone is afraid to talk about deviations, there is a tendency for them to be swept under the carpet, and that is certainly the last place we want to find the deviations! CAPAs should therefore never be accompanied by finger pointing. They should spur us on to roll up our sleeves and start working on being better next time.     Because honestly:  - When was the last time you innovated out of fear?  - When was the last time you fixed a systemic flaw by telling someone off?  - And don't we learn more by communicating openly and honestly than by trying to hide our mistakes?     It is an art to find the balance between the two extremes - celebrating every mistake on the one hand or penalizing every mistake on the other. Making mistakes should never become the norm, but an open and solution-oriented approach to mistakes and deviations will help the entire organisation.  What do you think? Take your time to prepare your study - otherwise watch out for the domino effect! Clinical trials are complex and multi-layered. It is not without reason that there are many guidelines, rules and regulations that need to be followed. Each task can be solved well or badly, and there are many decisions to be taken. For the second edition of our "Domino Effect"-post, we have selected the following 4 situations: - Unclear Eligibility Criteria - Inexperienced Trial Sites - Minor Protocol Deviations - Lack of Clear Stopping Criteria We present obstacles, associated risks and ways of risk avoidance.
