Yesterday afternoon, a friend and I were about 50 meters from a cardiac arrest event on the street 🚶♂️ My GoodSam app went off and we responded (luckily my friend is also an ALS instructor) 🚨 After a couple of rounds of CPR, an intensive care paramedic arrived with a defib - which was attached and we had a shockable rhythm (VF), shock delivered via AED, PEA/CPR, then ROSC. 🫀 The Ambulance Tasmania team were as 🆒 as cucumbers, highly organised, efficient, and coordinated. 12 MONTH ANNIVERSARY: It’s one year since NSW Ambulance launched the GoodSAM app and we are celebrating! Since launched, 39 lives have been saved thanks to registered volunteers who have stepped in to give CPR to someone having a cardiac arrest. The best chance of survival for anyone having an out-of-hospital cardiac arrest is receiving chest compressions and defibrillation from an automated external defibrillator as soon as possible. More than 8,000 people have registered as a GoodSAM volunteer and we can’t thank them enough. Signing up as a GoodSAM volunteer increases the odds that someone nearby can help save a life by performing simple chest compressions in the first few minutes while paramedics arrive . 📢 Is your EMS / ambulance service geared up to achieve first shock delivery in 6 minutes? A recent study in Circulation examines the impact of delays in delivering the first defibrillation shock for OHCA patients, specifically those with an initial shockable rhythm like ventricular fibrillation (VF). Analysing over 3,700 cases from the ARREST registry, the research reveals the profound effects of time on patient outcomes. 🔑 Key Findings: 6-Minute Window: When the first shock was delivered within 6 minutes, VF was successfully terminated in over 93% of cases. Delays beyond this window significantly reduced the chance of VF termination. Each Minute Matters: Every additional minute from the emergency call to the first shock was linked to a 6% lower chance of successful defibrillation, a 4% drop in return to organized rhythm, and a 6% decrease in the likelihood of survival to hospital discharge. Fewer Shocks, Better Outcomes: Patients who received timely defibrillation required fewer shocks overall and had better survival rates. 🚑 What This Means for Emergency Response: The study underscores the need to achieve a first shock delivery within 6 minutes, suggesting this as a potential performance benchmark for EMS and ambulance services. Investing in strategically placed AEDs and use of community responder apps such as GoodSAM can enhance response times and save lives. 🌐 Improving the Chain of Survival: As the study highlights, reducing the time to the first shock is a key driver in increasing OHCA survival. As we see with high performing EMS and ambulance services around the world. prioritising fast, coordinated responses as part of your cardiac arrest system of care can transform patient outcomes.
