Today, we have published our report and safety flyer on the investigation of the grounding and subsequent loss of the fishing vessel Ocean Maid (BA 55) on Cairnbulg Point, Aberdeenshire, Scotland on 24 October 2022. * Loss of grounded fishing vessel after watchkeeper became disorientated. * Formal training provides the knowledge and tools required to maintain an effective watch. * Make sure you take the opportunity to rest before taking a watch during the hours of darkness.

The Marine Accident Investigation Branch (MAIB) examines and investigates all types of marine accidents to or on board UK ships worldwide, and other ships in UK territorial waters. The sole objective of these investigations is to determine the circumstances and causes, with the aim of improving the safety of life at sea and the avoidance of accidents in the future. It is not the purpose to apportion liability, nor, except so far as is necessary to achieve the fundamental purpose, to apportion blame. We do not enforce laws or carry out prosecutions.

Today, we have published our report on the investigation of the capsize of the recreational craft Wheelyboat 123 with the loss of two lives on Roadford Lake, Devon, England on 8 June 2022. * Understand the needs of boat users, particularly those with additional vulnerabilities. * Make sure your boat is maintained in good condition, especially seals and openings. * Consider the capabilities of the person when choosing between a lifejacket and a buoyancy aid.

Today, we have issued a safety bulletin on the investigation of the foundering of fishing vessel Argos Georgia approximately 190 nautical miles east of Port Stanley, Falkland Islands on 22 July 2024, resulting in the loss of 13 lives. * Initial findings show flooding contributed to loss of vessel with 13 lives in the Southern Ocean. * Operators of fishing vessels should review the safety of side shell doors. * Operators of fishing vessels should review their flood control plans.

Today, we have published a report and flyer on the collision between the pair trawlers Guiding Light and Guiding Star that resulted in the flooding and sinking of Guiding Star. * Leaving the wheelhouse unattended can have catastrophic consequences. MGN 313 (F) requires that watchkeepers maintain a safe navigational watch, avoid distractions, and consider the impact of fishing operations on their primary responsibilities. * Be prepared for flooding emergencies. The Fishermen’s Safety Guide provides guidance to help crews develop flood action plans, and MGN 570 (F) offers drill scenarios to practice such plans. * Lifejackets and immersion suits must be readily accessible in case of abandon ship. MGN 570 (F) advises fishermen to consider suitable storage locations, and the risks associated with stowing such equipment inside the vessel.

MAIB’s new data portal goes live The MAIB Data Portal, designed to provide public access to a subset of maritime accident data, has launched. In development since August 2022, the data portal gives the public and industry open access to download and analyse MAIB’s data. The portal features a Power BI report dashboard, enabling users to filter data and capture results via screenshots. Additionally, MAIB has integrated the option to download a prepopulated Power BI PBIX file, allowing users to not only filter data but also export it into table format for further analysis. Where in the past users were required to submit Freedom of Information (FOI) requests, this service has simplified the process by allowing direct and immediate access to a subset of the data most requested. The data is then presented on a user-friendly dashboard as well as overlaid on a map. Alongside the interactive reports, raw data in CSV format is also available for the criteria: occurrences; vessels; affected persons. These tables are linked via unique keys and can be used with open-source analysis tools. Throughout the portal’s development, MAIB has collaborated closely with users and stakeholders from across the maritime industry to ensure the service meets their needs. As part of the project, MAIB has also enhanced its internal data quality tools, giving confidence in the accuracy and reliability of the data available via the portal. So far, data from January 2023 onwards is available via the portal; however, the dataset will gradually be expanded to offer a comprehensive resource through the dashboard. https://lnkd.in/e-w7jxFY

Today, we have published our accident investigation report into the investigation of the serious injury to a crew member on board the survey and supply vessel Kommandor Orca at Portland, England on 16 August 2022. * Unsafe operation of crane resulted in serious injury. * No local procedures allowed for unsafe crane operation. * Crane not operated in line with manufacturer’s instructions.

Today, we have published our accident investigation report into the investigation of a fatal man overboard from the sail training vessel Pelican of London at Sharpness, England on 2 October 2023. * In this tragic accident the relief cook fell from the top of the gangway into the water and drowned. * The gangway guard ropes had significant gaps of coverage and the safety net had not been rigged to catch a casualty. * The casualty was more than three times over the limit – alcohol and ships do not mix. * Policies, procedures, training and risk assessments need to mitigate foreseeable hazards.
