Transformation Partners in Health and Care (TPHC) offer high-quality bespoke consultancy services, and support to programmes and projects from neighbourhood to national levels, working with a range of partners from integrated care systems, local government, and NHS organisations. TPHC has an outstanding track record in achieving positive and tangible results for populations. In late 2022 we changed our name when our two teams - Healthy London Partnership and Healthcare Consulting came together as a single team called 'Transformation Partners in Health and Care'. You may still see references on this page to Healthy London Partnership in our older posts.

A great day today filming for our short video showcasing our programme supporting our care leavers across our five boroughs in North Central London, into jobs in the NHS. This collaboration between our local authorities, the voluntary sector and the NHS demonstrates the impact we can have across an ICS when you bring together a system-wide vision with place based design and delivery through local partners.
