About us
Founded in 2022, the Atrius Health Equity Foundation aims to make large-scale, strategic investments in selected neighborhoods in Eastern Massachusetts to turn the tide on the chronic conditions that cut short too many lives. These initiatives will be led by trusted, community-based organizations and will encompass the upstream causes of health inequities and strengthened connections to community-based primary care. We will meet the urgency of this moment with resources, talent and commitment to close the gap.

The Boston Community Health Needs Assessment (CHNA) Survey is live! The survey asks residents about their community health priorities, access to care, and availability of resources to support their health and well-being. This process is led by the Boston Community Health Collaborative and the Boston Public Health Commission. The survey closes in early December 2024.
