About us

Transforming the Culture of Care! Providing compassionate care begins with a secure, warm setting. At Azura Assisted Living and Memory Care we know that when our residents feel at home, they respond more positively and interact more with others. Our relationship-based MOSAIC care philosophy recognizes each person as an extraordinary individual and focuses on engagement. Azura Assisted Living and Memory Care and our parent company, Azura Living, have earned a reputation for being experts and elevating the standard of care with creative services, innovative therapies, and caring relationships. In fact, we have created our own evidence-based, cutting-edge training called MOSAIC to ensure that every team member has an enhanced understanding of our residents and those with memory loss, while recognizing how to personally communicate with and engage those within our family of care. We are blessed to have numerous assisted living and memory care communities in Wisconsin with continued expansions in the works! Please take the time to learn more about the community closest to you: Beloit, Brookfield, Clinton, Eau Claire, Fox Point, Kenosha, Manitowoc, Monroe, Oak Creek, Oconomowoc, Oshkosh, Sheboygan, Stoughton and Wausau!

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Eboney Britton, Assistant Executive Director at Azura of Brookfield - Elmbrook Home Eboney’s nominator shares, “Eboney had been such an amazing assistant ED. She is consistent in communicating with her families and building a trusting relationship with them. She is loving, kind and committed to Azura and our residents, and her love for her job shines through every day. Over the past few weeks, Elmbrook has had a few ladies that have been having difficult expressions. Eboney has taken it upon herself to bring them to her office and help her “file paperwork" while she is working on her computer. This has helped residents calm down and decrease their expressions. She is able to problem solve quickly and figure out when residents have expressions and what triggers them, and has taught her team different strategies to soothe them. I know her team really appreciates her commitment to helping them succeed as well as caring about the well-being of her residents. Thank you so much Eboney for being so loving and kind to your residents, team and families!”

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Noelle Corona, Quality of Life Coordinator at Azura of Fox Point Noelle’s nominator shares, “I’m writing to express the importance and impact of Noelle Corona’s efforts to help our mother, Wilma B. Boese, realize her dream — seeing her children’s book, Dirty Yellow Sneakers, in print — via Azura and the Dream Program. Mom has written children’s stories all of her life. She was encouraged by publishers, but never secured a publishing contract. Having Noelle produce her book with original illustrations (that she helped with) was undoubtedly a moving, exciting, and rewarding experience for mom. Watching the news segments, I recognized the joy and pride in mom’s face as she read what she could to the children at the school next door. I heard about the Dream Program when I was visiting mom at Azura, and I had no doubt as to her unfulfilled dream. As a published writer, professional editor, and former publisher, I knew it would take a special person to bring one of mom’s manuscripts to life with the love and care it deserved. Enter Noelle.” “When Noelle called me to talk about the project, I quickly became confident she would engage mom in making her dream come true in a meaningful way. My brother, Peter Boese, and I both feel Noelle did a stellar, heartfelt, and quality job with the manuscript as well as executing the ever-important step of bringing mom (and the fruit of her talents) to an audience — the school kids next door. Further, Noelle capitalized on mom’s birthday: It was the same date as the school event. What a day for Wilma! We knew Azura had an opportunity for publicity with mom and the book. Working with local television crews, Noelle balanced mom’s needs with the media’s, which was very important to us. We are grateful to Noelle for her professionalism, her understanding, and her sincere — and successful — efforts to make our mom’s dream a reality. At the age of 86, Wilma B. Boese is now a published author. Her voice has been heard. We love Dirty Yellow Sneakers! Mostly, though, we are happy for our mom. We wish Noelle future successes as she fulfills the dreams of people with dementia. What could be more rewarding?”

  • The Azura Team Member Dream Program is an internal culture program that fulfills the dreams of select team members each year. Similar to our MOSAIC Dreams program that fulfills the dreams of our residents, we recognized it was just as important to take care of our team, too. Valerie, our dedicated Quality of Life Coordinator at Azura of Oak Creek, has longed to get her scuba diving certificate. She loves going on adventures with her family and scuba diving solo has been on her bucket list for quite some time! Thanks to the Team Member Dream Program, Valerie and her husband became scuba certified and now enjoy many underwater excursions together. Thanks for your dedication to your team and residents, Valerie! Azura is lucky to have you! Team members, families and vendors can submit a dream for themselves or others through the form on our website, found here: https://lnkd.in/ghHkJUsk Dream big, team Azura!

  • Thank you for taking the tour, EUA team!

Frustrated. Anxious. Helpless. Overwhelmed. These are just a few of the emotions my colleagues and I experienced yesterday taking the Virtual Dementia Tour. For us, the experience lasted only a short period of time, and we could take off the equipment and resume our normal lives. For those suffering with dementia, this is their constant reality. My grandma suffered with dementia for the last few years of her life before passing in 2018. It was devastating for my family to witness her decline, and I can't even begin to imagine what *she* felt as the world around her became unfamiliar and unmanageable. This is why I choose to design for older adults. This is why I walk to champion a better future for those living with dementia. The Milwaukee Walk to End Alzheimer's is coming up on Sunday, September 15. Every year, I’m overwhelmed with emotions as thousands gather to commit their money, resources and voices to a brighter future for those living with dementia. It is so exciting to be in the era of treatment for Alzheimer’s, but there is so much work yet to be done. Join me this September or donate to the cause if you are able:  https://lnkd.in/dAXCB3K Huge thanks to the team at Azura Memory Care and Assisted Living who brought the Virtual Dementia Tour to the Milwaukee office of EUA for nearly 40 of our employees to experience. It meant so much to us and was a humbling way to start the week and ground our work in the experience of others. Check out Azura's page for opportunities to take the tour at one of their communities! #endalz #seniorliving #virtualdementiatour

  • Join us in congratulating our first group of Certified Dementia Specialists!! The Certified Dementia Specialist certification is 34 hours of learning in-depth information on different dementias, how to approach someone who is having behaviors and how to be successful in completing tasks with our residents while utilizing the resident's strengths & independence. Team members will also need to recertify on an annual basis to ensure their skills stay fresh. We are working towards certifying team members from all of our homes! These Dementia Specialits are the experts on dementia in their community, and we encourage families to turn to these team members when they need support or guidance with their loved one.

  • Core Value Winner Wednesday 💙 We are so excited to announce this quarter's Core Value Winners! Our team members have gone above and beyond, embodying the values that are at the heart of our company culture. Join us in congratulating... Marie Post-Perez, Executive Director at Azura of Sheboygan Marie’s nominator shares, “We have a challenging resident due to a language barrier. Often, he refuses care and his medication because he has a hard time understanding what is being asked of him. Last week, we were really struggling with him; each team member made an attempt but was left with no success. After chatting with Marie and sharing this resident’s journey with her, and how no one at Azura speaks his language, she was extremely innovative. Marie decided she was going to start a little "mens club" with the resident and two members of her own family who speak his native language. Yesterday, Marie's husband and her brother in law came in to visit with this resident. The visit definitely helped, as he did in fact end up taking his medication after some friendly conversation. Marie has already started to show innovation, compassion and love to the residents in Sheboygan. I am so happy to have her back with the Azura family!!!”

  • A #MOSAIC Dream Come True for Beth 🐴 What would your best day ever be? Would it be a day on the golf greens, meeting your favorite celebrity or sitting poolside? Azura Memory Care and Assisted Living asks every resident this very question, and connects with local businesses and people to make their dreams come true - this program is known as the MOSAIC Dreams program. In the years that the MOSAIC Dreams program has been around, Azura has had a resident dance at the halftime show with the Milwaukee Bucks Grand Dancers, fly in an airplane and a helicopter, golf with a world champ, and more! Beth is a resident at Azura of Stoughton, and grew up being a barrel racer! She owned many horses of her own, and worked on a dairy farm for most of her life. She loves horses, and has her room at Azura filled with pictures of them (and often can be seen wearing horse shirts and hoodies, too). We wanted to extend a huge thank you to Azura of Stoughton's caregiver, Kathye, for connecting us with her daughter, Amy, that provided the horses for Beth's dream!
