The Center to Advance Palliative Care (CAPC) is a national organization dedicated to increasing the availability of quality, equitable health care for people living with a serious illness. As the nation’s leading resource in its field, CAPC provides health care professionals and organizations with the training, tools, and technical assistance necessary to effectively meet this need. What is Palliative Care? Palliative care (pronounced pal-lee-uh-tiv) is specialized medical care for people living with serious illness. It is focused on providing patients with relief from the symptoms, pain, and stress of a serious illness—whatever the diagnosis. The goal is to improve quality of life for both the patient and the family. Palliative care is provided by a team of doctors, nurses and other specialists who work together with a patient's other doctors to provide an extra layer of support. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. Reducing Risks for Older Adults >> https://ow.ly/YaCr50TUMYr Did you know it is estimated that delirium goes undiagnosed in more than 50% of cases? This course provides context and best practices for systematically identifying older adults who are at risk for poor outcomes, including falls, delirium, and caregiving challenges when they are admitted to the hospital or surgery center. Learners will gain familiarity with the Institute for Healthcare Improvement’s Age-Friendly Health Systems framework, and will use the program’s 4Ms (What Matters, Medication, Mentation, Mobility) to practice harm reduction and age-friendly care. Special emphasis is placed on steps to assess for and reduce risks of delirium, and ways to prevent patient harm by addressing polypharmacy and reducing anticholinergic drug burden for the older adult.
