Central California Alliance for Health (the Alliance) is an award-winning regional Medi-Cal managed care plan that provides health insurance for children, adults, seniors and people with disabilities in Mariposa, Merced, Monterey, San Benito and Santa Cruz counties. We currently serve more than 442,000 members. Founded in 1996, the Alliance was established to improve access to health care for lower income residents. We have pursued this mission by linking members to primary care physicians and clinics who assist in managing their care. Our vision is: Healthy People. Healthy Communities. At the Alliance, we believe in providing care that supports the whole person. Our 2024 Impact Report highlights our dedication to investing in the health and well-being of our member communities, extending far beyond traditional healthcare. In 2024, we: • Invested $93M in health care initiatives and community organizations. • Funded $34M to permanent supportive housing projects, totaling 824 units. • Welcomed 38 Community health workers into our network, enhancing cultural competence and diversity to better reflect our members. • Began building our network of doulas, bringing in 12 new doulas in 2024. Those are just a few of the many milestones from 2024. 👏 As our CEO Michael Schrader said, "For our members to be able to obtain optimal health, they must first receive the support they need in other aspects of their life." Having a local presence in the communities we serve, so we understand the unique needs of these communities and our members, is integral to supporting our members. We are so honored to be featured in the Local Health Plans of California (LHPC) annual report, which highlights the significant community reinvestments that local plans are making across the state. These efforts go beyond traditional care, focusing on services that support the health and well-being of the whole person. 🌟 Here are just a few examples of how we and other local plans are making a difference: • Before launching our Community Supports program to provide members medically tailored meals, the Alliance piloted a two-year program aimed at reducing hospital readmissions. This initiative provided 14 nutritious meals per week for 14 weeks to 494 Medi-Cal members with serious illness and at high-risk for hospital admission. • LA Care Provider awarded $20 million to provide permanent supportive housing for people experiencing homelessness with serious health conditions. • Alameda Alliance for Health is partnering with First 5 Alameda County to connect children ages 3-5 years to well-child visits and support pediatric practices with improving screenings. • PARTNERSHIP HEALTH PLAN, INC. dedicated $9.3 million to clinic workforce recruitment since 2014. • IEHP (Inland Empire Health Plan) has supported three community resource centers in providing ongoing fitness classes, wellness workshops, health care and health plan information. In 2023, IEHP invested $2.8 million in its CRCs and served 55,000 visitors. These are just a few examples of how the Alliance and local plans across California are working to address social determinants of health and build healthier communities. 🎉 Exciting Opportunity for High School Students in Merced County! 🎬 We're thrilled to announce the Vax Facts Challenge, a video contest we created in partnership with the Merced County Office of Education (MCOE). This challenge encourages high school students to craft 30-60 second videos that will educate their peers and community on the importance of vaccines. Prizes include: - 1st Place: $500 gift card + $1,500 for the student's school. - 2nd Place: $250 gift card + $750 for the student's school. This is a fantastic opportunity for students to share their creativity while spreading critical public health messages. Whether through skits, interviews, or animation, students can highlight vaccine education and the importance of immunizations, with a special focus on vaccines required for school. 📅 Contest runs from January 27 - March 3, 2025.
