Led by Dr. Sherly Abraham, Clearway Health has been instrumental in supporting their Linkage to Care Program through emergency department screenings for hepatitis C. In just 4 short months, we've identified 11 patients without a primary care provider and are personally connecting them to PCP's within TBHC. In addition, our team worked with TBHC to enhance the patient registration experience, ensuring that patients are properly registered and can be seamlessly linked to care. Led by Dr. Mercedez Bernard, CCMH has made remarkable strides in improving patient care. She organized her team of residents to run the clinic and streamlined patient scheduling through Clearway Health outreach, allowing patients to be seen sooner. She also ensured emergency room physicians were confident that her team would provide follow-up care for hepatitis C-positive patients, removing key barriers to screening.
