Happy Case Management Week! Its National Case Management Week where case managers in all settings take time to celebrate the work they do in the complex healthcare system. This week we are celebrating our Case Managers at Concord Counseling Services. Our Case Managers ensure that our clients obtain access to the services they need. They also work on a daily basis in the community, they meet our clients where they are, and work toward enabling them to achieve increased levels of independence. We salute our Case Managers for all that they do!!
