  • The Dawn of FuSa Standards The mid-1970s saw two major industrial disasters that reshaped how we think about industrial safety. In 1974, the Flixborough disaster occurred in the U.K., resulting in an explosion that tragically claimed 28 lives and injured 34 more. A reactor circuit leak led to a massive release of flammable gas, which ignited, causing a catastrophic explosion. Just two years later, in 1976, the Seveso disaster struck in Milano, Italy, when a toxic chemical was accidentally released into the air, affecting over 500 nearby residents. The absence of an instrument to measure steam temperature was a key factor that led to the release. These tragedies highlighted critical gaps in safety protocols. As a direct response, the Seveso Directive (now Seveso Directive III) was enacted by the European Community, aimed at controlling major accident hazards. In the U.K., these regulations are referred to as Control of Major Accident Hazards (COMAH). These incidents serve as a reminder of the importance of rigorous safety measures in preventing industrial disasters. The lessons learned from Flixborough and Seveso continue to influence safety standards and practices to this day, emphasizing that proactive prevention is key to saving lives and protecting communities.     Congratulations to CSA’s, Richard S., on becoming a TUV-certified Functional Safety Engineer with TÜV Rheinland Group!    About the Certification:     The certification from TÜV Rheinland’s examination of the Hardware / Software Design according to IEC 61508 training acknowledges expert knowledge of the foundational functional safety standard, IEC 61508. The training addresses engineers who are responsible for the design and development of safety-related products and components. The objective of this training is to provide state-of-the-art knowledge in order to apply requirements of the international standard IEC 61508 effectively and in a goal-oriented manner. Furthermore the training provides the important and new aspects of IEC 61508 edition 2, especially how to handle soft errors.    Rich’s dedication and hard work have certainly paid off.
