Some health plans are eliminating benefits to get costs under control. But Ashish V. Shah says some innovative plans are asking: how do we use our home-based or non-medical benefits to really create value? Listen in to the “Improving Access & Transparency in Home-Based Services” podcast segment recorded at #AHIP2024 to hear more. Transitional care is the perfect time to evaluate members, raise awareness around the right types of benefits, and activate those benefits to stabilize patients, instead of pushing them into a traditional post-acute setting. “Decades of research has demonstrated there is no better time to engage patients in changing their health behaviors than throughout episodes of illness requiring hospitalizations and follow-up care,” said Mary Naylor, Transitional Care Model architect, and director of the NewCourtland Center for Transitions and Health at the University of Pennsylvania School of Nursing. Johns Hopkins School of Nursing and its Community Aging in Place-Advancing Better Living for Elders (CAPABLE) program, a person-directed, home-based program that addresses both function and healthcare expenses, bears this out. CAPABLE focuses on prevention and problem-solving, and building skills that participants can use in the future. Its research shows that it has provided more than six times return on investment. #preventativecare #valuebasedcare #seniorcare #aginginplace #healthcare #medicare

Forward-looking health plans are leveraging supplemental benefits as an integral part of their value-based care programs. The good news is that preventative interventions like ADL support, home safety evaluations, and healthy meal programs work when accessible and managed correctly. Research from Johns Hopkins School of Nursing and its Community Aging in Place-Advancing Better Living for Elders (CAPABLE) program shows that these types of benefits work to reduce health disparities, hospitalizations, and nursing home days. Most MA plans use the extra benefits they offer to attract new members and differentiate themselves from Medicare. That's nothing new. What is new is increasing pressure from the CMS to deliver those benefits in a high-quality, low-cost manner. But the environment to do that is challenging. But we have shown that there is value to be gained by more efficiently deploying benefits. Our new report “Rising Medical Expenses? How to Use Supplemental Benefits to Manage Total Cost of Care” lays out this approach. We take you through the steps to drive smart utilization of non-medical benefits to address your value-based care objectives. Demand for behavioral healthcare services has skyrocketed, reflecting a growing recognition of the importance of mental health. But health plans often struggle to provide adequate coverage due to complex network requirements, a shortage of qualified providers, and logistical barriers. A renewed commitment from CMS begins to address some of these issues. The right care coordination and network management tools can help you further improve the process. We cover 4 “must-haves” when evaluating tools to improve access to these important services. “Investing in the right solutions can help you engage a complex set of providers to quickly get your members the services they need,” said Travis Woyner. “For health plans, this leads to improved member outcomes, reduced costs, and a stronger, more compliant network.” Read more here https://lnkd.in/gMKqa6K4
