At HealthHelper, we recognize the profound impact that a longstanding relationship with a primary care provider can have on your health. Recent studies reveal that only about two out of five adults have been with their primary care provider for at least five years—a significantly lower proportion than in other countries. Why is this important? A continuous relationship with a primary care provider deepens trust, enhances personalized care, and is associated with lower overall healthcare costs and fewer hospitalizations. When your doctor knows your health history intimately, they’re better equipped to detect early signs of issues and tailor treatments that fit your unique needs. HealthHelper is committed to closing this care gap. Join us in fostering stronger patient-provider relationships. Together, we can build a healthier future where everyone benefits from continuous and compassionate primary care. #primarycare #valuebasedcare #populationhealth #HEDIS #qualitycare Source: Evan D. Natalie McGann, D.O. from TriValley Primary Care, PC shared a powerful moment that highlights why we do what we do: "I’m sitting with a patient who was 11 years overdue for her first mammogram. She shared with me, 'The reason I finally did it was because your office sent me a simple text. I have no idea why I could never make myself pick up that phone all these years, but you made whole process so positive and easy. It changed everything for me and I’ll never delay again!'" This is what it's all about—making healthcare accessible, convenient, and engaging for patients. A simple, positive message and service made all the difference for this patient. We're thrilled to know that our technology-enabled approach is helping close care gaps and improve lives. Here's to many more victories like this! 🙌 #HealthHelper #PatientEngagement #HealthcareInnovation #ClosingCareGaps #PatientCare #TriValleyPrimaryCare #ValueBasedCare #QualityCare

Exciting news in cancer screening! The FDA has approved Guardant Health’s blood test, Shield, for colon cancer screening. This is a major step forward in addressing the dismal colon cancer screening rates, with over 40% of people in the U.S. not up to date. Shield, a cfDNA blood-based test, has demonstrated an 83% sensitivity rate for colorectal cancer in average-risk populations. While colonoscopy remains the gold standard, having Shield as an option makes screening more accessible and convenient. Remember, the best test is the one that gets done. If you're interested in exploring your screening options, consult your primary care provider or gastroenterologist. At HealthHelper, we're dedicated to making healthcare more accessible and convenient. By supporting advancements like Shield with our partner practices, we aim to boost screening rates and improve health outcomes for all. #ColonCancerScreening #FDAApproval #PopulationHealth #ValueBasedCare #PrimaryCare #PreventiveCare #HealthcareInnovation https://lnkd.in/e2u3Quyq

CMS proposes a 2.8% cut to physician reimbursement. That’s the headline, and we don’t love it… but here are some of the lesser-mentioned positive proposals from CMS: -Advanced payments to MSSP ACOs with experience. That’s big for cash flow, which is a common pain point within the value-based care world! -Better incentives in MSSP for providers who serve rural and underserved communities. Let’s bring value-based care, and the wrap-around services it often includes, to patients in need! -Introduction of G2211 complexity code. Let’s reimburse more for managing longitudinal relationship complexity that isn’t captured in the work of standard E/M codes. -Creation of three new codes for advanced primary care management services that bundle elements of several existing care management and communication technology-based services. This could be the start of something big - truly paying for proactive care. The headline is disappointing, but there is a lot to be excited about beyond the headline! https://lnkd.in/exGxdp-s #primarycare #physicianreimbursement #CMS #valuebasedcare #populationhealth #ACO #MSSP
