    How Do I receive Hospice? Anyone can refer a patient to hospice service. One of our team members will visit you to assess your needs and work with your doctor to determine if you qualify for the benefit. Meeting with a hospice team is free of charge, and you will learn about the benefits you may be entitled to receive. We want you to have the knowledge you need to make informed decisions about your care. The hospice benefit is an elective benefit. You can stop it at any time and renew your traditional benefits. https://lnkd.in/gUgWduiX

    Is Hospice Care Right for Me? Hospice serves any patient who is clinically eligible for the benefit, including, but not limited to people living with Alzheimer’s disease, heart disease, COPD, stroke, Parkinson’s disease, ALS, and cancer.     Hospice provides four levels of care to meet your needs as your situation changes, including any hospitalizations: Routine home care: The basic level of hospice care −provided wherever you call home. Continuous care: The hospice benefit includes care in times of medical crisis – keeping you comfortable at home. Short-term respite care: When a family needs a break, the benefit covers care for up to five days in a contracted nursing facility or hospital. General inpatient care: Hospice provides care in a hospital, skilled nursing facility, or hospice inpatient unit for symptom control that cannot otherwise be provided at home.
