Our purpose is to reduce health inequities by finding and engaging high-risk, hard-to-reach individuals and equipping them to thrive. Our teams of dedicated care providers deliver telehealth-enabled, hands-on clinical care wherever individuals are, enable them to access social and other community-based resources, and bring guidance and compassionate support every step of the way. Our multidisciplinary approach is designed to untangle and address complex clinical, behavioral, and social needs and to create personalized, comprehensive care plans. We partner with Medicaid and Medicare plans and bring flexible, scalable solutions for improving member health and reducing inappropriate utilization.

At MedZed, we’re proud to celebrate Health Literacy Month, recognizing the importance of making health information accessible and ensuring that navigating the healthcare system is easier for everyone. Our dedicated Health Navigators and support staff work tirelessly to guide our members through their health journeys, making sure they can find, understand, and use critical health information to make informed decisions. By providing this support, we help improve health outcomes and ensure that those we serve have access to the care and resources they need.     As we celebrate Hispanic Heritage Month, MedZed is proud to recognize the incredible leadership of our Chief of Staff, Isabel Portieles, MBA, MA. Isabel is a dynamic design-thinker and problem-solver, bringing a wealth of experience across healthcare, medical devices, and health and wellness sectors. Her career spans from early-stage startups to developed companies, where she has played a pivotal role in building marketing functions from the ground up—driving branding, strategic planning, and campaign management.   In her role as Chief of Staff at MedZed, Isabel leads multi-disciplinary initiatives that streamline operations and drive strategic growth across the company. She ensures our teams stay aligned with MedZed’s mission to provide compassionate, high-quality care to underserved populations. Isabel’s academic journey is equally impressive, with an MBA from Johns Hopkins University, a Master’s in Design Thinking from the Maryland Institute College of Art, and an undergraduate degree from Yale University.   Isabel’s leadership at MedZed serves as a powerful example of what can be achieved with hard work, vision, and a commitment to making a difference. His career as a healthcare executive, sales leader, and social entrepreneur spans early-stage startups, Fortune 500 companies, and non-profits.   Enrique holds a bachelor's degree in economics and political science from Boston University and a Master of Public Policy from UC Berkeley, Goldman School of Public Policy.     At MedZed, we are proud to celebrate National Hispanic Heritage Month (September 15 - October 15) by honoring the rich history, cultures, and contributions of Hispanic and Latino Americans. We especially want to acknowledge the incredible work of our Hispanic staff members and the lasting impact they have on our mission to champion health equity in communities across the country. Hispanic contributions to medicine and healthcare are profound, with trailblazers like Dr. Carlos Juan Finlay, who discovered that mosquitoes transmit yellow fever, paving the way for advancements in public health. Dr. Helen Rodríguez-Trías, a pioneering Puerto Rican pediatrician and women’s rights advocate, was instrumental in expanding healthcare access for underserved populations. Dr. Antonia Novello, the first Hispanic woman to serve as U.S. Surgeon General, has made lasting contributions to public health policies. As we celebrate Hispanic Heritage Month, let’s recognize and honor the outstanding contributions of Hispanic individuals in healthcare, who continue to inspire and shape the future of medicine.     Today, we pause to honor and remember those who lost their lives on September 11, 2001, as well as the bravery and selflessness of the first responders, medical professionals, and everyday heroes who risked everything to save others.
