March is National Kidney Month, and at Monogram Health, we’re transforming how kidney care is delivered to those who need it most. Chronic kidney disease (CKD) often goes undiagnosed until it reaches advanced stages—many of the 200,000 patients we’ll serve this year won’t know they have CKD before our in-home care teams begin treating and educating them about their health. CKD rarely exists in isolation. Most patients also face conditions like heart failure, diabetes, and hypertension—chronic issues that accelerate kidney disease progression. Monogram is the only provider uniquely positioned to holistically treat both kidney disease and the conditions that cause it, offering a multispecialty, in-home approach that addresses the whole patient. By treating these underlying causes, we’re not only slowing disease progression, but also improving overall health and reducing hospitalizations. Starting this year, patients with chronic and end-stage kidney disease covered by Memorial Hermann Health Plan will gain access to Monogram Health’s high-quality, multispecialty in-home care.
