Paty Gomez has been an essential member of the Partners in Care Foundation family since 2019, contributing her expertise and dedication to the Enhanced Care Management (ECM) Program. Her role as a SUD Navigator is vital in supporting participants on their journey to recovery, helping them access critical services such as treatment programs, counseling, and community resources. Paty’s deep knowledge of substance use disorders, combined with her compassionate and empathetic approach, makes her a trusted advocate for those she serves. She works closely with participants to help them navigate complex systems, ensuring they receive the care and support they need. Her collaborative nature extends to her colleagues as well, as she is always ready to share her insights and provide support when needed. Paty skillfully manages her caseload, maintaining detailed and organized records that allow her to track participants’ progress and ensure timely follow-ups. Her strong organizational skills, coupled with her commitment to excellence, serve as a model for her team. Additionally, Paty is proactive in sharing best practices and valuable resources with her coworkers, fostering an environment of learning and mutual support. With her comprehensive understanding of the ECM Program and her passion for helping individuals overcome challenges, Paty is a true asset to our team. At Partners, our #BoardofDirectors understands that genuine care goes far beyond a diagnosis—it’s about engaging our participants with heartfelt conversation, active listening, and meeting them where they are. On our inaugural Board Field Day, each board member paired with a Care Team member for an Enhanced Care Management #ECM home visit, experiencing first-hand how personal engagement creates trust and transforms lives.
