About us

We believe everyone should have the opportunity to live their best lives and age with dignity. That’s why we’re reinventing the healthcare and aging experience for older adults and their loved ones through patient-centered care, coordination and navigation support that addresses total health and well-being. Our technology-enabled care model is centered around the unique needs and values of adults 65 and older, who deserve an experience that is human, personalized and easily accessible. We meet people on their terms, with comprehensive and convenient in-home and virtual care options that eliminate the frustration of rushed and transactional office visits. Each person we serve gets a dedicated care team built around them. Led by a primary care physician, the team includes a nurse practitioner, a health champion and specialized team members who can support the individual’s total medical, emotional, social and logistical care needs. Individuals can choose to make their family and loved ones part of their care team, too. We’re fiercely determined to improve the care experience for older adults. As we grow, we need more mission-driven people with the drive, passion, skills and audacity to help us change healthcare for good. Now’s a great time to join us!

    “This is such a different experience than I’ve had in the past. I feel like the whole team knows me and has my best interests at heart.” The Patina primary care experience puts people 65+ at the center of a team devoted to helping them live and age well. This experience comes to life in our care for Phyllis, a retired math teacher who leads an active life and strives to give back to others. A lifelong swimmer and exercise enthusiast, Phyllis suffered a stroke a few years ago and relied on her Patina care team to support her recovery – a genuine relationship that continues today as her health needs evolve.     “While these individuals face the same issues as many others who are aging – social isolation, loneliness, housing affordability, and ageism – research shows they have additional concerns that can take a toll on health and wellness.” – Marilynn Larkin, M.A., in an article in the Journal of Active Aging. By 2030, it’s estimated that there will be 7 million U.S. adults 65 and older who identify as LGBTQ, and the numbers this number will continue to grow exponentially – yet there is almost no information on aging in the community. What we do know is that older adults who are LGBTQ+ face many barriers when it comes to accessing healthcare, such as a history of discrimination, low rates of health insurance coverage, and disproportionate rates of HIV/AIDs, mental health issues, and chronic physical conditions. The LGBTQ+ community matters. Organizations like SAGE’s National Resource Center on LGBT Aging was created to address the unique needs of this vibrant and resilient community.
