October is #ADHD Awareness Month. A common question for dietitians is whether individuals with ADHD should avoid food additives. While some clinical trials have linked their consumption to a small increase in neurobehavioural effects, many others show no significant association and the presence of neurobehavioural effects has often been reported by parents but not by teachers. It would therefore be prudent to limit synthetic food dye in children's diets but given these discrepancies, the confidence in this recommendation should be low. If the child and their caregiver(s) are interested in trialing an elimination diet, dietitians should evaluate the possible risks, expenses and difficulty related to this intervention and consider whether it is realistic to implement. For critically ill adults, energy expenditure (EE) is best estimated by indirect calorimetry. When indirect calorimetry is not available for mechanically ventilated patients, EE can be calculated from VCO2 obtained by some ventilators or a weight-based equation (e.g.12-25 kcal/kg/day).
