⭐ Ask the Expert: Which Side to Perform CPR On? ⭐ Q: Should you position a dog on their right or left side for CPR? A: The RECOVER Guidelines are based on evidence. To our knowledge, no studies have looked at the difference between right and left lateral recumbency when performing external chest compressions. Because of this, there is no recommendation on which side the dogs should be positioned. It is more important to start chest compressions as soon as cardiopulmonary arrest is recognized in an animal and to perform them uninterrupted for 2 minutes. So, even if you believe that dogs should be right-side down for compressions, we recommend not delaying or interrupting compressions to turn your patient around. 🩺🐾 💬 Have you encountered this in practice? 🌟 Ask the Expert: Your Questions Answered! 🌟 Q: Does the RECOVER program have any recommendations for crash cart setup in large hospitals? A: Crash cart setup falls within Prevention & Preparedness. Currently, the main resource for prevention & preparedness is the 2012 P&P domain paper (https://lnkd.in/guF4uAr6). The 2024 Guidelines (https://lnkd.in/ewVn9c33) discuss equipment used in CPR; updated Prevention & Preparedness Guidelines are anticipated next year. Both articles are available open-access. There is also content available in: Advanced Monitoring and Procedures for Small Animal Emergency & Critical Care (2023, Wiley) - Chapter 2 ("The Small Animal Emergency Room") and Chapter 20 ("Cardiopulmonary Resuscitation") both by Smarick, both include lists for triage areas/crash carts. 🚨 Having the right setup can improve response times and patient outcomes. 📣Your Questions, Expert Answers! 📣 Q: Since hyperoxia can cause neurologic issues during recovery, I’ve understood that oxygen should be used only in O2-dependent conditions, while an Ambu bag with room air is sufficient for patients without respiratory issues. Is that correct? Also, what are the recommended oxygen flow rates? A: Great question! Here’s what we would recommend for CPR and Post-Cardiac Arrest (PCA) Care. During CPR, we recommend supplementing oxygen to maximize oxygen delivery to tissues. Because blood flow is severely decreased during CPR, maximizing the amount of oxygen in the blood during CPR is recommended, and connecting oxygen at a high flow rate to the resuscitation bag is recommended. During the PCA period, oxygen should be titrated based on the pulse ox or arterial blood gas to target a SpO2 (or SaO2) of 94-98%. If higher than 98%, the flow rate should be decreased, and if < 94% it should be increased. This is to ensure adequate delivery of oxygen to the tissues but reduce the risk of hyperoxia, which can induce free radical production and worsen ischemia-reperfusion injury. 💡 Have a question for our experts?
