It’s been a big year for developments in metastatic breast cancer (mBC), with a host of scientific advances and new treatment options offering hope to patients. As we move beyond #MBCAwarenessDay, here are six of the best from 2024, according to our team of experts: Claire Gillis, Jason Gloye, Nichole Davies, Susanne Bobadilla, PhD, Sarah-Jane Barker and Alexandra Lewis. #1. Neoadjuvant immunotherapy (Keytruda) followed by chemotherapy improves overall survival in patients with advanced triple-negative breast cancer; median overall survival for patients with a PD-L1 combined positive score of 10 or more was 23 months for those who received Keytruda + chemo, versus 16.1 for those who received chemo alone. #2. Antibody-drug conjugates (ADCs) could become the new standard of care in mBC; Enhertu shows substantial and durable efficacy in patients with HER2-positive metastatic breast cancer, including those with brain metastases. It’s also an effective new treatment option for some metastatic HER2-low and -ultralow breast cancers, yielding better PFS and response rates than chemotherapy. #3. Verzenio improves progression-free survival in certain metastatic breast cancers; People with metastatic hormone-receptor-positive, HER2-negative breast cancer who received Verzenio plus Faslodex lived longer without the cancer growing than those who received Faslodex alone. #4. Inavolisib received Breakthrough Therapy Designation for PIK3CA-Mutated, HR+/HER2– Breast Cancer. It’s since been granted a priority review for a new drug application, with an FDA decision due next month #5. Triple combination could be a good option in HR+/HER2-negative PIK3CA-mutant breast cancer. The combination of Itovebi, Ibrance and Faslodex reduces the risk of progression or death in patients with PIK3CA-mutated hormone receptor-positive HER2-negative mBC. #6. Oral SERDS are set to impact treatment of hormone receptor-positive advanced breast cancer. Orserdu, already approved in second line for patients with ESR1-mutant advanced breast cancer, significantly improves progression-free survival compared to traditional endocrine therapies. Additional next generation SERDs are in development for first-line metastatic breast cancer in combination with CDK4/6 inhibitors to maximize endocrine therapy.
